Objective:Objective to observe the effect of massage therapy on early prevention of deep venous thrombosis after total hip arthroplasty.Objective to explore the mechanism of massage therapy for deep venous thrombosis,and to provide evidence for clinical application.Method:Methods a total of 60 patients with unilateral primary total hip arthroplasty as a result of non fracture disease from October 2015 to December 2016.All the patients according to the treatment time were randomly divided into intervention group(n=30)and treatment group(n=30),There were 21 males and females(n = 39),Their mean age(59.43±4.64)years.The following treatment plan:(1)The basic treatment group was given conventional treatment:the patients were all operated by the same group of doctors,and the operation was performed by placing the drainage tube in the operation for about 24 hours after operation,not more than 48 hours,Postoperative antibiotics to prevent infection in 24 hours,the conventional analgesic and anti-inflammatory treatment.After 12 hours of operation,oral anticoagulation with Rivaroxaban tablets,10mg/d,course of treatment for 5 weeks.Instruct patients to carry out routine exercise.(2)intervention group:At the same time,the treatment group was treated with the lower limb massage manipulation,20min/times,2 times/day on the first day after operation,and then to the operation for 2 weeks.Two groups of observation index:1.determination of D-dimer in venous blood;2.postoperative VAS pain score;3.lower extremity color Doppler ultrasound thrombosis;4.popliteal vein blood flow changes;5.postoperative thigh circumference measurement.Observe and record the two groups before and after the operation of each stage of the relevant evaluation index,get data,and using SPSS software for statistical analysis.Results:(1)There was no significant difference in the general data between the two groups(P>0.05);(2)Comparison between the two groups after operation:first days and fourth days after operation,There was no significant difference in the level of D-dimer(P>0.05).Seventh days,tenth days and fourteenth days after operation,The level of D-dimer in the intervention group was significantly lower than that in the treatment group,There was statistically significant(P<0.05);First days and fourteenth days after operation,There was no significant difference in VAS score(P<0.05),Fourth days,seventh days and tenth days after operation,VAS score in the intervention group was lower than that in the basic treatment group,There was statistical significance(P<0.05).First days after operation,There was no significant difference in deep venous thrombosis formation rate between the two groups,There was no statistical significance(P>0.05),Seventh and fourteen days after operation,The incidence of deep venous thrombosis in the intervention group was lower than that in the basic treatment group,There was statistical significance(P<0.05),First days after operation,No significant difference between the two groups of subjects with double popliteal vein blood flow velocity,There was no statistical significance(P>0.05),Seventh and fourteenth days after operation,The intervention group double popliteal vein blood flow velocity is the basis of the treatment group is high,There was statistical significance(P<0.05).First days after operation,No significant difference between the two groups of size around the thighs,There was no statistical significance(P>0.05),Fourth days,seventh days and fourteenth days after operation,The thigh circumference of the intervention group was smaller than that of the basic treatment group,There was statistical significance(P<0.05).Conclusion:Massage therapy can effectively prevent the formation of deep venous thrombosis of lower limbs after unilateral total hip arthroplasty,and contribute to the early postoperative swelling of the lower limb.
