| Objectives: Through clinical observation,data collection,are retrospectively analyzed and studied the clinical curative effect of Wen-Yang-tong-mai granule(WYTMG)with conventional western medicine treatment of cerebral ischemic diseases(The spleen kidney Yang deficiency type cerebral infarction),for WYTMG of arteries and veins in the dialectical treatment,as well as the traditional Chinese medicine combined disease are many links,many ways,on multiple targets for treatment advantage to provide the basis for further.Methods: A total of 60 patients(60 cases of cerebral infarction)with cerebral ischemic stroke disease(spleen kidney Yang deficiency type)coming from the first affiliated hospital of anhui university neurology of traditional Chinese medicine was divided randomly into treatment group and control group(each group of 30 cases).All cases were randomly set up WYTMG with conventional treatment(treatment group)and routine treatment group(control group),and were randomized controlled trials,two groups of a course of 4 weeks.Before and after 2 weeks,4 weeks treatment the effects of hemorrheology,the coagulation index and the transcranial doppler(TCD)were respectively detected,at the same time the degrees of nerve function defect score(CSS)and the changes of TCM syndrome integral value were obsevsed before and after therapy.All the observed results were for statistical analysis.Results:(1)clinical efficacy evaluation:The recovery-apparent rate of 86.7? and the total effective rate of 96.7? on Chinese medical syndrome in the treatment group with WYTMG was obviously higher than the control group.The ratio of the latter were 60.0? and 73.3?(p < 0.01 or p <0.05);Treatment group in clinical symptoms,signs and TCM syndrome score than the control group were significantly improved(p< 0.01 or p < 0.05),especially in reducing neural function defect score compared with the control group significant difference(p <0.05);Clinical markedly effective rate and total effective rate of treatment group with WYTMG(66.7?,90.3?)were higher than the control group(36.7?,80.0?),the markedly effective rate of clinical significant difference compared with controls(p <0.05).(2)clinical index evaluation:(1)In blood rheology indexes,treatment group with WYTMG can obviously reduce the parameter values.The whole blood viscosity,plasma viscosity,erythrocyte electrophoresis,and fibrinogen of all the examination indexes compared with the control group were decreased significantly(p < 0.01 or p < 0.05).(2)In coagulative function indexes,the treatment group can obviously prolong PT,APTT and TT.Particularly the PT and the APTT are significant difference(p < 0.01)from controls.(3)In transcranial ultrasound blood flow diagram(TCD)examinations,treatment group blood flow velocity(Vm)and arterial parameters(PI)no significant difference compared with control group(p > 0.05).(3)Safety index:During the period of treatment and follow-up,two groups of patients to take drug adverse reaction observed did not appear harmful response.After treatment the security indexes of the two groups in the three routine,liver and kidney function,electrolyte,and electrocardiogram(ECG)compared with before treatment had no statistical difference(p > 0.05).The results of the above show WYTMG with western medicine had no adverse effect on patient safety indicators,and the clinical applications are safe.Conclusion : WYTMG with Conventional Western medicine treatment has better clinical curative effect of ischemic stroke patients.At the same time of improving symptoms,the blood rheology and blood coagulation index have a positive impact than pure western medicine.Its mechanism maybe Chinese traditional medicine by many links,many ways,and multiple targets acting on the complex pathological links in cerebral ischemic diseases,and lowering the body's high coagulation state,improvingmicrocirculation,presented certain curative effect and unique advantage.Although the treatment in improving effect of Vm and PI(TCD indexes)was not significant,but the affirmative curative effect further confirmed the advantages of Chinese medicine dialectical treatment and the complexity of the role.So this treatment is worth clinical popularizing and using. |
