Purpose:To investigate the relationship between the changes of alpha-fetoprotein and prognosis before and after embolization of hepatic artery in patients with primary liver cancer.Method: The clinical data of 117 patients of our hospital with primary hepatocellular carcinoma treated by TACE were analyzed retrospectively.The changes of alpha-fetoprotein concentration were divided into three groups: group A(42 cases)TACE(AFP decreased more than 20%);group B(40 cases)TACE after a change in alpha-fetoprotein value(AFP decreased or increased by no more than 20%);group C(35 cases)increased the level of alpha-fetoprotein after TACE(alpha-fetoprotein increased by more than 20%).The disease control rate,survival curve difference and 2-year survival rate were compared between the three groups.Results:Group A DCR = 95.24%,Group B DCR = 77.50%,Group C DCR = 48.57%.The difference of control rate between the three groups was statistically significant(?~2 = 22.472 P <0.001).Disease control rate: Group A> Group B> Group C.The difference of control rate between group A and group B was statistically significant(P <0.001).The difference of control rate between group A and group C was statistically significant(P <0.001)Meaning(P = 0.009).The median survival time was 21 months,the survival rates were 95.2%,84.7%,71.4% and 35.7% at 3,6,12 and 24 months respectively.The median survival time of group B was 15 months,,6,12,24 months survival rates were 90.0%,80.0%,57.5% and 10.0%;C group of patients with median survival time of 12 months,3,6,12,24 months survival rates were 88.6 %,74.3%,40.0% and 5.71% respectively.The difference between the two groups was statistically significant(P = 0.008).The difference between group A and group C was statistically significant(P <0.001).There was no significant difference between group B and group C(P = 0.085).2-year survival rate: Group A> Group B> Group C.There was a significant difference in the 2-year survival rate between group A and group B(?~2 = 7.610,P = 0.006).There was a significant difference in the 2-year survival rate between group A and group C(?~2 = 9.987,P = 0.002).There was no significant difference in the 2-year survival rate between group B and group C(?~2= 0.668,P = 0.679).Conclusion: 1.TACE is an effective method for the treatment of advanced liver cancer patients;2.Primary liver cancer patients before and after TACE changes in the situation can predict the prognosis of the situation;3.TACE postoperative AFP decreased prognosis Good;4.TACE after AFP no significant changes suggest poor efficacy;5.TACE after AFP elevated suggestive of poor prognosis.
