Objective:To investigate current situation of negative emotions(e.g.anxiety and depression)and the quality of sleep status during in patients with coronary heart disease.And to explore the clinical effect of respiratory biofeedback therapy on improving these negative emotions and sleep quality,so as to provide scientific basis for nursing work in the future.Methods:All subjects were recruited from patients who were hospitalized from October 2015 to October 2016 in the department of Cardiology,Yijishan Hospital,Wannan Medical College,A total of 115 patients who met inclusion criteria were selected by using the Hospital Anxiety and Depression Scale.They were randomly divided into group A and group B.There were 55 cases in the group A and 60 cases in the group B.The group B was given routine nursing care of the cardiology department during the hospitalization.In addition to the routine nursing care,the group A were also treated in respiratory biofeedback therapy with Spirit-8 multi-channel biofeedback instrument every other day.The self-developed general questionnaire,the hospital anxiety and depression scale and Pittsburgh Sleep Quality Index scale were used for evaluating the baseline and improvement of patients in related index on the first day and the last day of hospitalization respectively.Then the differences of anxiety and depression,sleep quality and autonomic nerve function before and after treatment were compared,meanwhile,comparison in the cardiovascular events and hospitalized days between the two groups was also made.Results:? A total of 180 patients with coronary heart disease received questionnaire survey.The proportion of patients whose Anxiety scale or Depression scale scores ?8 points accounted for 63.8%.The sleep quality of most enrolled patients in the study was impaired,that the propotion of subjects whose weight score of sleep ?7 points were accounting for 84.0%in all enrolled patients.? After treatment,both negative emotions(e.g.anxiety and depression)related problems and the overall quality of sleep in the two groups were improved,which expressed as that the hospital anxiety and depression scale and Pittsburgh Sleep Index scale scores were significantly reduced(P<0.05).After treatment,the improvement rate of group A was significantly higher than the group B,performance of subjective sleep quality and sleep time of patients in group A were significantly better than group B(P<0.05).After treatment,the Hospital anxiety and depression scale score(11.56±1.61,13.97±1.49),Pittsburgh Sleep Index scale total score(7.44±1.94,8.61±2.16),subjective sleep quality score(0.97±0.43,1.25±0.43)and sleep time score(1.05±0.31,1.32±0.47)of patients in group A were significantly lower than the group B,(P<0.05).? Compared with baseline,the SDNN value of patients in the group A increased(50.79±42.75,53.95±42.27)but which decreased in group B after treatment(39.60±12.88,30.93±7.79),the difference was statistically significant(P<0.05).?The incidence of cardiovascular events in the group A was 23.3%,and the corresponding value in group B was 43.9%,the difference was statistically significant(P<0.05).The hospital days of patients in the group A and group B were 10.28±3.813 and 9.53±3.586 respectively,and the difference between two groups was not statistically significant(P<0.05).Conclusion:?More than half of hospitalized patients with coronary heart disease are accompanied with anxiety or depression,these patients generally exist symptoms of sleep disorders.?Respiratory biofeedback therapy can help improve the anxiety or depression(especially anxiety)in patients with coronary heart disease,and the subjective sleep quality and sleep time of these patients might be ameliorated through this therapy.?Respiratory biofeedback therapy can improve HRV(SDNN)in patients with coronary heart disease.This therapy can also reduce complications of hospitalized patients,but can not shorten hospitalized days of these patients.
