Objective(1)Emotion management training program based on health belief model was constructed.(2)To explore the influence of emotional management training program on the emotional regulation ability,emotional state and sleep quality of patients with coronary heart disease.Method1.The construction of emotional management training programLiterature research:searching domestic and foreign databases is to analyze the design and situation of emotional management or emotional regulation intervention,which provides reference for the construction of emotional management training program in patients with coronary heart disease.Group discussion:After discussion by research group,the intervention content,intervention form,intervention time and intervention staff of emotional management training program were determined to form the first draft of the emotional management training of the coronary heart disease.Cross-sectional survey:From November 2018 to March 2019,279 patients with coronary heart disease were screened in four hospitals of Zhengzhou,and the participants were investigated using cognitive emotion regulation questionnaire.Semi-structured interviews:Eight patients with coronary heart disease were conducted by semi-structured interviews,extracting relevant content through the interview data is to revise the emotional management training program of patients with coronary heart disease.Expert consultation:The 12 experts from geriatric medicine,rehabilitation medicine,cardiology nursing,clinical psychology were screened to make importance judgment and provide modified suggestions for emotional management training program.The enthusiasm and authority of experts were assessed by the familiarity,judgment and authority scores on the intervention program.Preliminary test:Seven patients with coronary heart disease were randomly selected to test emotional management training program.The intervention program was reviewed according to the patients' feedback and problems in the intervention.2.Application of emotional management training programA total of 94 patients with coronary heart disease who met the inclusion and exclusion criteria were screened as participant in Grade 3A hospital in Zhengzhou,they was divided into the intervention group(48 patients)and control group(46 patients).Patients who were assigned into the control group received regular health education(basic health guidance,push health knowledge by rehabilitation assistant).Patients who were assigned into the intervention group received regular health education and emotional management training program based on the health belief model.Cognitive emotion regulation questionnaire,emotion management scale,hospital anxiety and depression scale,and Athens Insomnia Scale were used to evaluate the effect of emotional management training program on mental health of patients with coronary heart disease at baseline,after intervention and 1 month after intervention.Results1.Construction of emotional management training program(1)Literature research:based on the logical framework of support group program of cancer caregiver designed by Fan professor,the intervention mode,interventions time,intervention purpose and content,grouping method,evaluation indicators and evaluation time were initially extracted by the content analysis method.(2)Expert consultation:the two rounds of experts response rate were 91.7%and 100%.The familiarity coefficient and authoritative coefficient of the expert group was 0.933 and 0.929 respectively.(3)The emotional management training program based on the health belief model was constructed by literature review,preliminary survey,semi-structured interviews,two rounds of expert consultation and preliminary trials.The intervention duration was 6 weeks,a total of 7 times,3 times face-to-face interventions and 4 times rehabilitation assistant Internet intervention.The theme of intervention programs include Sensing emotional temperature—recognizing emotional diversity,controlling emotions skills—being the master of emotions,positive cognition—experiencing well-being,shaping positive mindset—experiencing owning,paying attention to rehabilitation plan—growing with you,experiencing benefits—acting with you.2.Application of intervention program:Of the 112 patients,18(16.07%)were lost due to sudden death,transfer to hospitals,and interrupted intervention.A total of 94 patients with coronary heart disease finished the whole research.(1)Comparison of cognitive emotion regulation scores after intervention in two groupsThe results of the generalized estimation equation of cognitive emotion regulation scores for patients with coronary heart disease showed that there is significant difference in group effect,time effects,and interaction effects of dimensions(except for desire to accept).Comparison of scores of cognitive emotion regulation in intervention group at different time points found that the scores of maladaptive emotion regulation after intervention were lower than those before intervention,and the scores of adaptive emotion regulation were higher than those before intervention(P<0.05).There was no significant difference in the scores of cognitive emotion regulation after intervention and 1 month after intervention(P>0.05).(2)Comparison of scores of emotional management after intervention in twogroupsThe repeated measurement of variance analysis showed that there is statistical significance in group effect,time effects,and interaction effects of emotional management.Comparison of scores of emotional management in intervention group at different time points found that the scores of emotional management scores after intervention were higher than those before intervention(P<0.05).There was no significant difference in emotional management scores after intervention and 1 month after intervention(P>0.05).(3)Comparison of scores of anxiety and depression after intervention in twogroupsThe repeated measurement of variance analysis of the anxiety and depression scores showed that there is significant difference in group effect,time effects,and interaction effects of anxiety and depression.Anxiety and depression scores in intervention group were statistically significant at different time points.That is to say,the scores of anxiety and depression changed with time,and the change trend was different(P<0.01).(4)Comparison of scores of insomnia after intervention in two groupsThe repeated measurement of variance analysis of insomnia scores showed that there is statistical significance in group effect,time effects,and interaction effects of insomnia.Insomnia scores in intervention group was statistically significant at different time points.That is to say,the scores of insomnia changed with time,and the change trend was different(P<0.01).Conclusion(1)Emotion management training program experts based on health belief model have high authority and good enthusiasm.Expert opinions suggested that the intervention program has clinical significance for promoting psychosomatic health of patients with coronary heart disease.(2)The emotional management training program based on the health belief model are easy to operate.It can effectively promote the positive transformation of patients' negative coping,improve emotional management ability,reduce negative emotion and insomnia among patients with coronary heart disease.
