| Objective: To study the correlation between cerebral microbleeds(CMBs)and cognitive function in adults with silent cerebral small vessel disease,and find the risk factors for CMBs.Cerebral small vessel disease are common in the elderly.According to the clinical symptoms,to divide odjiections into symptomatic cerebral small vessel disease and non symptomatic cerebral small vessel disease,the latter refers to silent cerebral small vessel disease.Cerebral small vessel disease imaging diagnosis including lacunar infarction and lacunar lesions,white matter hyperintensities,cerebral microbleeds and cerebral atrophy.This kind of disease often do not appear motor or sensory dysfunction.Thus can not draw enough clinical attention.Cognitive dysfunction is even more difficult to find in the early stages of the disease.So the diagnosis rate is very low,resulting in the late treatment effect is poor,affect the quality of life of patients seriously.Bring heavy burden to family and society.With the clinical application of SWI,more and more micro bleeding was found,and it has aroused neurologists ’ s special attention.At present,the correlation between cerebral microbleeds(CMBs)and cognitive function has become a hotspot.To study the correlation between cerebral microbleeds(CMBs)and cognitive function in adults with silent cerebral small vessel disease,and find the risk factors for CMBs.It is of great clinical significance to intervene and control some risk factors.If we could explore correlation between cerebral microbleeds(CMBs)and cognitive function and to find the risk factors for CMBs as soon as possible.Therefore,our study selected patients with silent cerebral small vessel disease as the research object,to study the correlation between cerebral microbleeds(CMBs)and cognitive function,and find the risk factors for CMBs.Methods: A total of 96 consecutive non-acute stroke patients without neurological disorder in Neurology of the First Affiliated Hospital of Wannan Medical College who had undergo SWI using a 3.0-T system,which was used to detect CMBs.And doctors collected the patients ’ s age,sex,admission systolic blood pressure,diastolic blood pressure,fasting blood glucose,glycosylated hemoglobin,total triglyceride,total cholesterol,high-density lipoprotein,low-density lipoprotein and so on.Detailed inquiry the past medical history,such as coronary heart disease,hypertension,diabetes.Head MRI+MRA+SWI examination was conducted if meeting our condition,and according to the existence of cerebral microbleeds,into two groups.Cerebral microbleeds positive group recorded the occurrence of cases,the number of microbleeds,location,record the number of lacunar infarction,leukoaraiosis situation.MMSE scale was used to evaluated cognitive function.Record the score of MMSE ll data were statisticsed by software SPSSl9.0.Results:1.Ttwenty one of the 80(26.3%)participated had at least one microbleed.Compared with cerebral microbleed positive group and the control group,no significant difference in age,sex,blood lipids,coronary heart disease,diabetes mellitus.But there was statistically significant difference of MMSE score between cerebral microbleed positive group and non-cerebral microbleed.MMSE score of cerebral microbleed positive group was lower than that in the control group.2.The difference of blood pressure grade was no statistical difference.But there was significantly difference of presence of leukoaraiosis,lacunar infarction,the severity of leucoaralosis and lacunar infarction was significantly higher than that in non cerebral microbleed group.3.Comparison of MMSE Score in CMBs Positive and CMBs negative.CMBs was weakly associated with Orientation,attention and calculation.However,an association of CMBs with other cognitive domains,including Working memory,delayed recall and language,was found.4.Grouped according to MMSE score,The mean age of normal group was lower than that of the lower group.But they did not differ significantly in gender,hyperlipidemia,coronary heart disease,and diabetes mellitus.5.MMSE score is significant correlation of presence of leukoaraiosis,lacunar infarction.There was a negative correlation between the MMSE score and the presence of leukoaraiosis,lacunar infarction(r=-0.378,P=0.01).Namely,the heavier of lacunar infarction and leukoaraiosis,the lower the MMSE score.6.Multivariate logistic regression analysis showed that cognitive function was significantly associated with CMBs,leukoaraiosis,and lacunar infarction.Conclusion:1.CMBs are independent risk factors for cognitive impairment.2.CMBs is associated with MMSE score,which was strongly associated with working memory dysfunction,delayed recall dysfunction and language dysfunction. |
