Objective:The aim of the present study was to assess the predictive value of the preoperative platelet lymphocyte(PLR)and neutrophi llymphocyte rates(NLR)on the Lymph node(LN)metastasis in colon cancer patients?Thus to to guide the lymph-adenectomy in Colectomy.Patients and methods:A total of 239 operable patients with colon cancer were enrolled in our study.There were 137 males(57.3%)and females(42.7%),with an average age of 58.30 years(range,aged from 27 to91).Leukocyte count,Neutrophil count,lymphocyte count and platelet count were collected?The clinical utility of the PLR and NLR was evaluated by receiver operating characteristic(ROC)curves.The number of lymph nodes was collected and the lymph node metastasis rate was calculated.The cases were divided into two groups by the threshold value.The relationship between preoperative NLR?PLR and clinical pathology was analyzed.Univariate and multivariate analysis of factors associated with lymph node metastasis.Results:1.The ideal cut off values for predicting LN metastasis were 1.99 for NLR and 144.43 for PLR according to the ROC curve.2.The preoperative NLR and PLR were significantly correlated with the tumor size,tumor size,differentiation,histological type,lymph node stage,depth of invasion,TNM stage,vascular invasion.3.The tumor size,size,differentiation,invasion depth,vascular invasion,perineural invasion,NLR,PLR were significantly correlated with lymph node metastasis.Multivariate analyses showed that both PLR and NLR are associated significantly with LN metastasis independent of depth of invasion,macroscopic type.Conclusion:1.Preoperative NLR and PLR of colon cancer patients can be used to forecast lymphnode metastasis,identify lymphnode staging to make preoperative diagnose more accurate.2.NLR and PLR is an important factor to evaluate immune function and also a convenient biomarker for cancer state.and increased NLR and PLR suggested that the clinical stage of tumor was late.The clinical application is convenient,economical,practical and easy to Operate.3.Preoperative NLRand PLR are quite helpful to make the dissection range and amount clear to avoid excessive ordeficiency of lymphnode dissection.4.its mechanism may be useful for cancer prevention and treatment. |
