Objective: To explore the application value of 3.0T perfusion-weighted MR imaging(PWI)in preoperative diagnosis and grading of brain gliomas.Methods : The PWI features of 12 cases of single brain metastasis and 30 cases of gliomas(14 cases of low grade,16 cases of high grade)were analyzed according to the pathological results.The conventional MR imaging and PWI were performed in all cases before operation.The time-signal curve and rCBV value in the tumor parenchyma area and the peritumoral edema area were calculated and statistically analyzed.Results: In the tumor parenchyma area,the rCBV value of single brain metastasis was lower than high-grade gliomas(4.04±0.84 vs.5.52±1.33,P<0.05).The rCBV value of low-grade gliomas was lower than high-grade gliomas(2.28±0.76 vs.5.52±1.33,P<0.05).In the peritumoral edema area,the rCBV value of single brain metastasis was lower than high-grade gliomas(0.73±0.34 vs.1.45±0.28,P<0.05).The rCBV value of low-grade gliomas was lower than high-grade gliomas(0.69±0.19 vs.1.45±0.28,P<0.05).It was showed that the time-signal curve was different between single brain metastasis and high-grade gliomas,and between low-grade gliomas and high-grade gliomas.Conclusions: Our data suggest that PWI has a high clinical valuein preoperative diagnosis and grading of gliomas.
