Part One The study of Dynamic Contrast-enhanced MRI and Diffusion Tensor Imaging:identify benign and malignant nodules in prostate central zone Objective:To investigate the diagnostic utility of dynamic contrast-enhanced magnetic resonance imaging?DCE-MRI?,diffusion tensor imaging?DTI?and two techniques combined for benign and malignant nodules in prostate central zone.Materials and Methods:60 cases patients with abnormal signal nodules in prostate central zone,which underwent magnetic resonance imaging with T2 WI,DTI and DCE scan..In the 89 central zone nodules,38 PCa nodules and 51 BPH nodules were confirmed with biopsy-proven.The values of FA,ADC,the peak time?Tmax?,maximum signal strength?Slmax%?,enhancement ratio?R?were measured,SI-T curve were discribed respectively on the nodules,then compared the differences in the parameters between PCa,BPH of,and ROC curve analysis.Results:The values of FA,ADC,Tmax,R between PCa and BPH groups were statistically significant?P=0.000,P<0.05?differences between the groups were not Slmax% statistically significant?P=0.223,P>0.05?;PCa group SI-T curve at washout of signal intensity type,BPH platform-type-based group.The area under ROC curve?RUC?for DCE,DTI and two techniques combined was 0.87?95% CI0.751-0.942?,0.85?95%CI0.734-0.933?,0.94?95% CI0.837-0.983?,respectively.Conclusion:Diagnostic evaluation for benign and malignant nodules in prostate central zone of DTI is higher than DCE.Combining two techniques provides better diagnostic accuracy.Part Two The correlation between DTI quantitative parameters and Gleason scores of malignant nodules in prostate central zone Objective:To investigate the relationship between ADC,FA value,the quantitative parameter of diffusion tensor imaging?DTI?of central gland prostate cancers,and Gleason score.and to investigate the predictive value of DTI in grading of prostate carcinoma.Materials:38 malignant nodules in prostate central zone were confirmed with biopsy-proven.All patients underwent magnetic resonance imaging with DTI scan,The ADC value and FA value of the nodules in prostate central zone were measured.The central gland prostate cancer were classified into Gleason scores?6,Gleason scores=7,Gleason scores?8,three group,according to the result of pathology.The ADC value and FA value of the central gland and the three groups Gleason score were analyzed by one way ANOVA,every two groups was compared.Person correlative analysis test of The ADC value and FA value of the central gland prostate cancer and Gleason score was used.Results:The mean ADC value of three groups malignant nodules in prostate central zone was?1.373±0.308?×10-3mm2/s,?1.002±0.20 9?× 10-3mm2/s and?0.746±0.195?×10-3mm2/ s,respectively.Themean FA value of three groups malignant nodules in prostate central zone was 0.375±0.281,0.301±0.231 and 0.196±0.231 respectively.The ADC value and FA value was statistical difference among three groups regions?P<0.05?.The ADC values and FA values in cancerous regions were negatively correlated with Gleason score?ADC value r=-0.760,P<0.05;FA value r =-0.687,P<0.05?,the higher the Gleason score was,the smaller the ratio was.Conclusion:There is a significant negative correlation between the ratio of ADC and FA of the central gland prostate carcinoma and Gleason scores,DTI is helpful for prediction of the central gland prostate carcinoma grade.
