Part ? Value of 3.0 MR Diffusion-Tensor Imaging in the diagnosis of peripheral prostate cancerObjective To explore the diagnostic values of diffusion tensor imaging?DTI?in peripheral prostate cancer.Methods Retrospectively,analyzing the imaging materials of 60 prostatic diseases with 25 peripheral prostate cancer?malignant group?and 40 benign prostate hyperplasia?BPH?and/or chronic prostatitis?CP?patients?benignant group?,which included conventional MRI,diffusion-weighted Imaging?DWI?and diffusion tensor imaging?DTI?,and then biopsy proven.Region of interest were chosen to measure the ADC vlaues and the FA vlaues.Receiver operating characteristic?ROC?curves were used to compare the diagnostic efficiency with ADC vlaues,FA vlaues and the combination.The cutoff of ADC values and FA values were determined.Results The mean ADC values and FA values of the cancerous and noncancerous regions were?0.89±0.19?×10-3mm2/s,0.25±0.05 and?1.46±0.23?×10-3mm2/s,0.17±0.04,respectively.There were statistically significant differences in ADC values and FA values between the cancerous and noncancerous regions?t values were 10.414 and-7.789 respectively,P<0.05?.Using the ADC value of 1.135×10-3mm2/s and the FA value of 0.196 as the ROC cutoff points,the sensitivity and specificity for the diagnosis of prostate cancer were 96.0%,95.0% and 96.0%,85.0%,respectively.The AUC of the ADC values,FA values and the combination were 0.974,0.907 and 0.990,95%CI were 0.9000.998,0.8090.965 and 0.9261.000,respectively.Conclusion Both the ADC value and FA value provide valuable information to the diagnosis and differentiation of PCa.The AUC for ADC+FA was significantly higher than that for either ADC or FA alone.Part ? 3.0 MR Diffusion-Tensor Imaging combined with Dynamic Contrast-enhanced MRI in the diagnostic value of peripheral prostate cancerObjective To explore the diagnostic value of DTI combined with DCE-MRI in peripheral prostate cancer.Methods Referring to the classification criterion of DCE-MRI on prostate imaging reporting and data system version 2?PI-RADSv2?,the DCE-MRI materials of the 65 patients were observed and classified.With the previous threshold of FA and ADC values used in the diagnosis of peripheral PCa in Part 1,the included 65 cases were reclassified.When the ADC value ? 1.135 × 10-3mm2/s or FA value >0.196 were taken as positive,the opposite negative.For the two combinations,the number of positive was greater than or equal to one,the result was positive.Nevertheless,for the three combinations,the number of positive was greater than or equal to two,the result was positive.Receiver operating characteristic?ROC?curves were used to compare the diagnostic efficiency with ADC,FA,DCE-MRI and the combinations.Result The sensitivity and specificity for the diagnosis of prostate cancer of ADC?FA?DCE-MRI and ADC+DCE+MRI?FA+DCE-MRI?ADC+FA+DCE-MRI were 96.0?95.0,96.0?85.0,72.0?77.5,96.0?72.5,96.0?62.5 and 92.0?100.0,respectively.The area under curve?AUC?and 95%CI were 0.955?0.905?0.747?0.842?0.793?0.960 and 0.8730.991,0.8060.964,0.624 0.847,0.7310.921,0.6740.883,0.8800.993,respectively.Conclusion ADC +DCE+MRI has better diagnostic efficiency than the FA+DCE-MRI.ADC+FA+DCE-MRI possesses the highest value in the diagnosis of peripheral PCa.
