The Study On The Changes Of Glypican 3-CTL In Peripheral Blood Of Patients With Hepatocellular Carcinoma After GPC3-CTLs

Objective:Application of gelatin sponge particles by transcatheter arterial chemoembolization(GSMs-TACE)in the treatment of hepatocellular carcinoma(HCC),and discuss the effect of the method on the frequency and percentage of GPC3 specific cytotoxic T lymphocyte(GPC3-CTLs)in peripheral blood of patients with hepatocellular carcinoma.Methods:11 patients with HCC were treated with GSMs-TACE,5 patients were performed surgical resection as control group,and 9 healthy persons were selected as healthy control group.Tumor markers(AFP,PIVKA-?)and various biochemical examination were tested on 4d,10 d,20d before and after GSMs-TACE,and midsection CT scan were reviewed on 4d,midsection enhanced CT reviewed on 30 d.2m L peripheral venous blood was collected from HCC patients before and 10 d,20d after TACE.2m L venous blood from the normal control group was collected once.The frequency and percentage of GPC3-CTLs in peripheral blood were detected by HLA-A genotyping method,Tetramer technology and Flow cytometry.Results:Flow cytometry was applied to detect the frequency and percentage of GPC3-CTLs in peripheral blood.Compared with the normal control group,the frequency and percentage of GPC3-CTLs after GSMs-TACE treatment group increased significantly,the difference was statistically significant(p<0.05).Change in the frequency and percentage of GPC3-CTLs before and 10 d,20d after GSMs-TACE was statistical significance(p<0.05).The GPC3-CTLs frequency and percentage of 10 days after GSMs-TACE was significantly higher than that before operation,and the peak value appeared.After 20 days,although the index decreased,but still higher than preoperative level.Change in the frequency and percentage of GPC3-CTLs before and 10 d,20d after surgical resection was statistical significance(p<0.05).The frequency and percentage of GPC3-CTLs decreased gradually.The value of AST/ALT and TBIL was increased 4d after TACE,and then gradually declined to the normal range.There was no significant difference in the value of ALT and TBIL 10 d,20d after GSMs-TACE compared with preoperative(p>0.05).There was no significant difference in the value of AST 20 d after GSMs-TACE compared with preoperative(p>0.05).There was no significant difference in the value of ALB 4d,10 d and 20 d after GSMs-TACE compared with preoperative(p>0.05).Compared with preoperative,the level of AFP and PIVKA-?4d,10 d and 20 d after GSMs-TACE decreased significantly,and showed a progressive downward trend,the difference was statistically significant(p< 0.05).Conclusion: After GSMs-TACE,HCC patient increased the frequency and percentage of GPC3-CTLs in peripheral blood,and it was preliminary confirmed that the GSMs-TACE method could activate the endogenous anti-tumor specific immune response;GSMs-TACE can promote the GPC3-CTLs frequency and percentage of peripheral blood in patients with HCC,which may be associated with this method causes a wide range of tumor necrosis.
