Study On The Changes Of Dendritic Cell And The Related Cytokines In Hepatocellular Carcinoma After Gelatin Sponge Particles Transcatheter Arterial Chemoembolization

Objective:To observe the changes of dendritic cell(DC) and the related cytokines(CK) of IL-10, IL-12and clinical index in hepatocellular carcinoma(HCC) after350-560μm gelatin sponge particles(GSPs) transcatheter arterial chemoembolization. Then to explore the meaning of DC changes and study the relationships between DC,CK, and clinical indicators.Methods:According to the selection criteria of the experiment, we choose30HCC patients as the treatment group, with the treatment of use350-560μm GSPs transcatheter arterial chemoembolization only. Moreover select15cases of healthy subjects and15posthepatitic cirrhosis cases as control group. All of the patients have the check of AFP and biochemical inspection before the day of the preoperative and after postoperative for4days,7days and30days. At the same time,after the consent of the patients written consent, take venous blood in the preoperative and postoperative4days,7days,30days. Flow cytometry(FCM) was used to detect the DC phenotypic CD la, CD83expression. By the method of ELISA to measur IL-10, IL-12content.Result:1. For the posthepatitic cirrhosis and HCC patients, the expression of CD la and CD83are lower than normal group(P<0.05), IL-12content are lower than normal group(P<0.05), IL-10content higher than normal group(P<0.05).2. For the treatment group, the expression of blood CD la and CD83are significantly increased(P<0.05) after TACE for4days,7days and30days compared to the preoperative peripheral, but the levels of the30th day is lower than the7th day, so no statistical significance(p>0.05). After treatment for4days,7days,30days IL-12content are significantly higher than before(P<0.05), increased gradually. After TACE for4days,7days and30days the IL-10content of the treatment group all declined obviously, have statistical significance(P<0.05).3. There is negative correlation between IL-10content and CD la, CD83expression. There is a positive correlation between IL-12content and CDla, CD83expression. 4. After TACE for4days,7days and30days, AFP are markedly decreased(P>0.05). ALT, AST, TBIL are improved significantly (P<0.05), after postoperative for4days, compared with4days, the7days and30days are declined(P<0.05), and after7days,30days and before operation have no significant difference(P>0.05). After4days,7days,30days and before operation ALB have no significant difference(P>0.05).5. The expression of CD la, CD83have no relationships with age, gender, tumor size, tumor number and BCLC staging(P>0.05).Conclusion:1. HCC patients have the symptom of DC phenotypic not mature, functional defects, immune function inhibition.2.350-560μm GSPs transcatheter arterial embolization of hepatocellular carcinoma can improve the number and function of DC, thereby may induce the immunity. Tumor necrosis may stimulate the immune reaction.
