Feasibility Study On Dual-source Dual Energy CT Predicts Suitable Stone Components For Extracorporeal Shock Wave Lithotripsy

Abstract/Summary:
Objective :The aim of this study is to use dual source CT stone analysis technology to analyze the upper urinary stones,based on the results of the analysis of infrared spectroscopy.Probe into the difference of dual energy parameters of different components of stones.In order to predict the clinical effect of extracorporeal shock wave lithotripsy for upper urinary calculi.Materials and Methods: A total of 81 upper urinary stones from 76 patients,which were obtained by open surgery or other treatment means,collected from June 2015 to September 2016.All patients underwent preoperative DSCT scanning.Infrared spectroscopy was used to measure the composition of these stones.Urinary stones were classified as calcium oxalate monohydrate stone group(n = 14),mixed calcium stone group(n = 56),ammonium magnesium phosphate stone group(n = 4),cysteine stone group(n = 2),uric acid stone group(n = 5),based on the results of infrared spectroscopy and the degree of fragility of different kinds of stones in extracorporeal shock wave lithotripsy.Hounsfield units(HU)of each stone were recorded for the 100 kV and the 140 kV datasets by hand-drawing method.HU difference,HU ratio and DEI were calculated.Using variance analysis to compare 100 kV HU,140 kV HU,HU difference,HU ratio and DEI of different stones groups.Result:1.Compare HU difference,HU ratio and DEI in different groups,there were statistical differences in DEI between each group(P <0.05);there was no statistically significant differences in HU difference between calcium oxalate monohydrate stones and mixed calcium stones(P >0.05);there was no statistically significant differences in HU difference and HU ratio between magnesium ammonium phosphate stones and cystine stones(P >0.05);there were statistical differences in HU difference,HU ratio and DEI between the other groups(P <0.05).2.Compare 100 kV HU and 140 kV HU in different groups,there were statistical differences between calcium oxalate monohydrate stones and cystine stones、uric acid stones(P <0.05);there were statistical differences in 100 kV HU and 140 kV HU between mixed calcium stones and uric acid stones(P <0.05);there were statistical differences in 140 kV HU between mixed calcium stones and cystine stones(P <0.05);there were statistical differences in 140 kV HU between in phosphoric acid ammonium magnesium stones and uric acid stones(P <0.05);there was no statistical differences between the other groups(P >0.05).Conclusion:Dual-source dual-energy CT has the ability to differentiate UA stones,cystine stones,mixed calcium stones,phosphoric acid ammonium magnesium stones and calcium oxalate monohydrate stones in vivo.
