Objective:To evaluate the diagnostic yield of EUS-FNA for lesions adjacent to the gastrointestinal tract.Methods:By retrieving the digestive endoscopy database of The Second Hospital of Hebei Medical University,screening a total of 22 patients(12 men,10 women,age from 34 to 81)with lesions adjacent to the gastroint-estinal tract operated by endoscopic ultrasound guided fine aspiration from June 2014 to December 2016.These patients were diagnosed as lesions adjacent to the gastrointestinal tract by ultrasound,CT or MRI.8 of the 22 were mediastinal masses,13 of the 22 were pancreatic lesions(11 solid lesions,2 cystic lesions),and 1 was celiac lesion.The puncture samples of all patients were processed by histopathology,liquid-based cytology or smears.Suspected tuberculosis lesions were sent acid-fast staining and TB-DNA.The cystic fluid of pancreatic cystic lesions were sent CA-199,CEA and CA-125.The pathologic results,postoperative complications,late treatment and prognosis were tracked after EUS-FNA.Results:1 EUS-FNA was a safe and effective method in diagnosing the lesions adjacent to the gastrointestinal tract with rare complications.2 The overall diagnosis sensitivity,specificity,positive predictive value,negative predictive value and accuracy of EUS-FNA for distinguishing malignancy from benign lesions adjacent to the upper gastrointestinal tract were respectively 88.9%,100%,100%,66.7% and 90.9%.3 The diagnostic accuracy of EUS-FNA with ?3 passes was consistent with >3 passes.The diagnostic accuracy of EUS-FNA in pancreas was consistent with mediastinum.4 The diagnostic accuracy of various processes with EUS-FNA samples such as histopathology,liquid-based cytology or smears,were equal.Conclusions: EUS-FNA was a safe,effective and accurate method for distinguishing malignancy from benign lesions adjacent to the upper gastrointestinal tract with rare complications.The number of passes,location and sample processing does not affect the diagnostic accuracy.
