Objective To establish an evaluation index system of occupational protection for clinical nurses that accords with the current status of Chinese oncology,to provide a scientific and systematic evaluation standard in the field of occupation protection management of the hospital in order to promote the sustainable development of the standardization.Methods To conduct operation management theory,with a combination of literature review and expert interview,after the sufficient discussion of team members,the initial development of 3 first level general hospital chemotherapy occupation protection evaluation index system framework.Twenty experts of relevant fields were enrolled and expert consultation was conducted using Delphi method.The consultation results were evaluated by positivity,authority,degree of opinion concentration and degree of opinion coordination.50 clinical nurses were selected from a 3 first level general hospital who have chemotherapy drugs practiced in the clinical more than 1 year by convenience sampling method.Self-assessment scale based on the constructed system was answered by the 50 clinical nurses to test the reliability and validity of the evaluation system.Result 1.20 experts from related fields completed two rounds of the consultation which the average age of(44.75 ± 6.11)years.The active coefficients were 100.0% and 95% respectively and the authority coefficients were 0.875 and 0.882.2.In the first round of the evaluation system,there were 57 indexes.The arithmetic mean of importance score was 3.344~4.867,the standard deviation was 0.388~0.918,the number of indexes with CV<0.2 was 47(82.5%).And Kendall's W coefficient was 0.208.In the second round of the evaluation system,there were 80 indexes.The arithmetic mean of importance score was 3.484~4.976,the standard deviation was 0.373~0.778,the number of indexes with CV<0.2 was 77(96.25%).And Kendall's W coefficient was 0.214.In the finally evaluation index system,including 3 first-grade indexes,12 second-grade indexes and 55 third-grade indexes was established.3.Precedence Chart method and Proportional distribution method were used to set the weights at all levels value,the first-grade indexes are individual factor(0.425),the weight of item factor(0.300),and the safety management of hospital and department(0.275).4.The evaluation index system was established in the form of questionnaire,50 questionnaires were issued with 98% effective rate.The average score of self-protection of 50 nurses was(68.03±10.09).The statistical results manifested that Cronbach coefficient of the system was 0.837,The Cronbach's alpha coefficients of three first-grade indexes were 0.834,0.848,and 0.819 respectively.Retest correlation coefficient was 0.805,CVI=0.902.Conclusion 1.Experts have better authority and representation,a higher degree of concern for this study and familiarity with the content of the evaluation system,and result was reliable.2.Because of the combination of Delphi consultation and precedence chart,the indicators and their weights setting results of index system cover content were more comprehensive,scientific and accuracy.3.The results show that the evaluation index system has good reliability and validity,which can objectively evaluate the status quo of occupational protection of chemotherapeutic drugs and be a certain operational and policy guidance.It can be used as an evaluation tool for clinical nurses in the field of occupational protection management.
