A Factor Analysis Of Non-sIn Lymph Nodes Metastasis In Breast Cancer Patients With Positive Axillary Sentinel Lymph Node Biopsy

ObjectiveThrough retrospectively analyzed the clinical data of patients with breast cancer,which Sentinel Lymph Node Biopsy(SLNB)results were positive and Axillary Lymph Nodes were performed(ALND)at the same time,analyzed its related factors,to provide a reference for further treatment of breast cancer patients whose lymph node biopsy results are positive.Methods and MaterialsSelect 97 cases of patients with breast cancer,who were treated by Fujian province tumor hospital between January 2005 and December 2016,which sentinel lymph node biopsy(SLNB)results were positive and Axillary lymph nodes were performed(ALND)at the same time.Put the 46(47%of the overall)patients whose Postoperative pathology confirmed non-sentinel lymph node,(NSLN)was transfered as the experimental group,and the rest 51(47%of the overall)patients whose NSLN wasn't transtered as the control group.Select patient's age,menstrual status,the size of the Primary lesions,the lump quadrant,the positive and negative number of Axillary sentinel lymph node,the pathologic type,Organization classification,Vascular invasion,Estrogen Receptor(ER),Progestrone Receptor,(PR),Human epidermal rowth factor receptor 2(HER-2),Proliferating cell nuclear antigen(Ki-67)and other factors as the reference,by single factor and multiple factors analysis,find out the related risk factors.ResultsSingle factor analysis results are as follows:Tumor size(P=0.002),the positive number of Axillary sentinel lymph node(P=0.030),the negative number of Axillary sentinel lymph node(P=0.002)and Organization classification(P=0.021)have statistical significance In the distribution difference between transfer and no transfer group of NSLN.While the patient's age,menstrual status,the Primary lesions site,the pathologic type,ER,PR,HER-2,Ki-67 and Tumor emboli situation Within the vascular have not statistical significance In the distribution difference between transfer and no transfer group of NSLN.Multiple factor analysis results show that:the size of the Primary lesions(P=0.002?OR=2.352),the number of positive sentinel lymph node in the cleaning out(P=0.030?OR=3.457),Organization classification(P=0.021,OR=4.027)Positively related to the Axillary sentinel lymph node metastasis,while the number of Negative sentinel lymph node in the cleaning out Negatively related to the Axillary sentinel lymph node metastasis.ConclusionsOnly a fraction of patients,whose Sentinel lymph node biopsy for positive results,Axillary sentinel lymph node positive(about 47% In this study),The primary breast tumor size,the positive number of Axillary sentinel lymph node and Histologic classification are the Independent risk factors of Axillary sentinel lymph node,while the number of Negative sentinel lymph node are the Protective factors of Axillary sentinel lymph node.
