Study On The Difference Of Gleason Score And Pathological Staging Between Prostate Biopsy Specimens And Radical Resection Specimens

Objective:The objective of this paper is to investigate the difference between Gleason score of preoperative biopsy and radical prostatectomy specimens,and to identify its influencing factors.Method:A retrospective analysis of 120 patients diagnosed as prostate cancer by transrectal ultrasound guided biopsies and subsequently underwent radical prostatectomy from January 2012 to December 2015 at our hospital was performed in this study.Preoperative serum PSA levels,prostate volume,PSAD,and Gleason score of biopsy,the number of positive biopsy cores,and other relevant variables,including Gleason score,positive surgical margin,seminal vesicle invasion and lymph node metastasis of radical prostatectomy specimens were collected.The difference between Gleason score of preoperative biopsy and radical prostatectomy specimens were examined by Chi-square test.Univariate and multivariate logistic regression analysis were used to determine predictors of Gleason score difference.Results:Gleason score of postoperative specimens was consistent with that of preoperative biopsy in 70 cases(58.3%),higher in 41 cases(34.2%),and lower in 9 cases(7.5%).According to Chi-square test,the number of positive biopsy cores,prostate volume and biopsy score were associated with the difference in Gleason score(p<0.05).According to univariate analysis,PSAD,the number of positive biopsy cores,prostate volume and biopsy score were associated with Gleason score difference(p<0.05).According to multivariate analysis,prostate volume and biopsy score were the independent predictors of Gleason score difference(p<0.05).Conclusion:There was difference between Gleason score of biopsy and prostatectomy specimens.Prostate volume was the independent predictor of Gleason score difference,we should pay close attention.
