| BackgroundIn accordance with the definition of ’configuration and use management approach of large-scale medical devices’ of the former Ministry of Health and ’use evaluation system of large-scale medical devices(Trial)’of the National Health and Family planning Commission,large-scale medical devices refers to A and B large medical devices which are included in the management items of health administrative department.But the definitions of large-scale medical devices from various hospitals are different.Larger hospitals are often defined with a unit price of 500,000 yuan,while smaller hospitals are defined with a unit price of 200,000 yuan.According to the requirements of ’medical devices management approach of medical and health institutions’ of the former Ministry of Health,’medical devices whose unit price being 500,000 and above,should make a feasibility argumentation.’ A large-scale medical device was defined with a unit price of more than 500,000 yuan in this paper.With the development of science and technology,more and more large devices have entered the hospitals,providing reliable guarantee for hospital operation efficiency and social benefit.However,the attendant problems have emerged.Nowadays,hospitals are faced with a problem that is how to solve scientific introduction and effective dynamic management of large-scale medical devices.The solution to this problem is that hospitals must make the early demonstration and decision analysis of large-scale medical devices before invested,and the benefit analysis after used.ObjectiveTo analyze and evaluate the working conditions of large equipment,take measures to improve its utilization,guide the planning and project of hospital medical equipment and use the limited hospital funds to the most efficient projects,the hospital has carried out the operation efficiency analysis and evaluation on the large clinical charging equipment costing more than 500,000 yuan for several years in accordance with the relevant assessment requirements of review of Grade Ⅲ Class A hospitals and annual evaluation and the evaluation indicators and methods of ’use evaluation system of large-scale medical devices(Trial)’of the National Health and Family planning Commission.On this basis,the paper went a step further to analyze the impact of the medical services price adjustment of large-scale equipment on hospital in comprehensive medical reform during six months before and after the implementation of the medical reform and focus on the four key equipments of hospital.MethodsThis paper selects a Grade Ⅲ Class A hospital,which has a longest history,largest amount of assets and highest of cognition and satisfaction degree of the general public in Jinan.Combining with the author’s practical experience in the hospital,the study will use the principles of economics and statistics to find the difficulties of benefits analysis.By adopting the commonly used cost-benefit evaluation indicators,investment payback period and investment rate of return,the study mainly analyzed economic benefits of large clinical charging equipment costing more than 500,000 yuan in 2016 and the use of large equipment in the last three years from 2014 to 2016.ResultsThe research showed that the institutions were set up reasonably and the division of labor was clear.The management system of asset life cycle has been established.The level of economic benefit analysis of large medical devices has increased year by year,and the data obtained has been more and more close to real data.In the 274 clinical devices that could be statisticed benefit in 2016,there were 120 devices whose investment payback period being under 3 years,accounting for 43.80%of the number of statistical revenue devices.There were 30 devices whose investment payback period being between 3 to 6 years,accounting for 10.95%of the number of statistical revenue devices.There were 124 devices whose investment payback period being over 6 years,accounting for 45.25%of number of statistical revenue devices.A comprehensive healthcare reform was implemented from June 2016.Since then,the impact of medical service price adjustment on the benefits of large-scale device in the hospital was not obvious.The greater the value of key equipment,the better of benifit.Conclusion and suggestionsThe economic benefits of large-scale medical devices in this hospital showed polarization status.There are still deficient in asset management and equipment benefit analysis in the hospital.We should give a help for the hospital to find out reasons and make effective recommendations to avoid the blindness of equipment purchase.Eliminating the phenomenon of forcing on purchase,overlooking use and ignoring management could ensure the improvement of use efficiency of large medical devices and the reduction of operating costs. |
