| Objective1.To observe whether the axial length of type 2 diabetic patients is related to the occurrence of diabetic retinopathy(DR).2.To observe the correlation between the axial length of type 2 diabetic patients and the occurrence of diabetic macular edema(DME)3.To investigate whether the axial length can be used as a predictor of the development of DR and DME in patients with type 2 diabetes,and provide clinical evidence for the prevention of DR and DME.Methods102 patients with a diabetes duration of 10-15 years from May 2017 to December 2017 in our department of endocrinology and ophthalmology were selected.The length of the axial of the affected eye was measured using the IOL master and the interquartile range was used.The affected eyes were divided into four groups:(Q1: 20.16-22.89 mm,Q2: 22.89-23.63 mm,Q3: 23.63-23.98 mm,Q4 23.98-30.45 mm).Dilated fundus photographic examination,using ETDRS classification of patients with fundus lesions were divided into: no DR(ETDRS = 10),mild DR(ETDRS = 20-35),moderate DR(ETDRS = 43-47),severe DR(ETDRS=53)PDR(ETDRS≥61).DME is classified into: no DME,mild DME,moderate DME,and severe DME.The measured data were statistically analyzed.Results1.According to the severity of DR,the affected eyes were grouped into 5 groups.The axial length of each group was compared: no DR group(24.20±0.56mm),mild DR group(23.40±0.32mm),moderate DR group(22.93±0.37mm),severe DR group(22.50±0.39mm)and PDR group(21.93±0.34mm),we found that the more serious the DR,the shorter the axial length of the patient,the difference was statistically significant(P<0.01).According to the severity of DME,the affected eyes were divided into 4 groups.The axial length of each group was compared.There were no DME group(24.09±0.50mm),mild DME group(23.15±0.33mm),moderate DME group(22.69±0.38mm)and the severe DME group(21.66±0.31mm),the more severe the degree of DME was,the shorter the axial length of the patient was,and the difference was statistically significant(P<0.0125)2.According to the severity of DR,the affected eyes were divided into 5 groups and the depth of anterior chamber between each group was compared.There were no DR group(3.30±0.56mm),mild DR group(3.45±0.60mm),moderate DR group.3.33±0.55mm),severe DR group(3.27±0.52mm)and PDR group(3.45±0.49mm),there was no significant difference between each group(P>0.01).According to the severity of DME,the affected eyes were divided into 4 groups.The depth of anterior chamber was compared between each group.There were no DME group(3.32±0.54mm),mild DME group(3.40±0.55mm),moderate DME group(3.30±0.56mm)and severe DME group(3.33 ± 0.68mm),there was no significant difference between each group(P> 0.0125).3.After adjusting for gender and age factors,we found a negative correlation between axial length and DR(OR=0.657,95% confidence interval 0.436-0.738,P < 0.05 AL,Per 1mm increase).By comparing the correlation between the axial length and the severity of DR,it was found that Q4 in the longer axial group had mild DR(0R=0.367,95% confidence interval was 0.199-0.676,P<0.05),moderate DR(0R = 0.323,95% confidence interval was 0.163-0.638,P<0.05),Severe DR(0R=0.358,95% confidence interval was 0.061-0.409,P<0.05)and PDR(0R=0.300,95% confidence interval was 0.072-0.557,P<0.05)were smaller than those of the shorter axial group Q1.4.After adjusting for gender and age,we found a negative correlation between axial length and DME(OR=0.519,95% confidence interval was 0.354-0.897,P < 0.05,Per 1mm increase).By comparing the correlation between the axial length and the severity of DME,it was found that the long axial group Q4 group had mild DME(0R=0.527,95% confidence interval was 0.123-0.622),moderate DME(0R=0.287,95% confidence interval was 0.092-0.578)and severe DME(0R=0.211,95% confidence interval was 0.038-0.327)was less than Q1.Conclusions1.Long-axis is a protective factor for DR,and patients with longer axial lengths have lower rates of mild DR,moderate DR,severe DR,and PDR than those with short axial length.2.Long-axis is a protective factor for DME,and patients with longer axial lengths have lower rates of mild DME,moderate DME,and severe DME than patients with shorter axial length.3.There was no significant correlation between ACD and DR,ACD and DME. |
