Objective1.By analyzing the data of adult HIV/AIDS patients,to understand the epidemic characteristics of adult HIV/AIDS patients in recent years,and to provide reference for AIDS prevention and control measures.2.To analyze the incidence,clinical features and risk factors of liver injury in adult HIV/AIDS patients,establish early warning mechanism for high-risk groups,formulate individualized treatment plan,improve patient compliance to treatment,provide a theoretical basis for the smooth treatment of HARRT.Methods1.Collect HIV/AIDS data in 2010-2015 from the first hospital of Changsha AIDS clinic to save the adult HIV antiviral treatment record of medical records-the basic situation,follow-up and medication table,Ecxel2007 software was used to establish the patient database,and SPSS20.0 software was used to analyze the data.2.A retrospective analysis of 2010-2015 years in the first hospital of Changsha outpatient HARRT treated adult HIV/AIDS patients with clinical manifestations and laboratory tests,statistics the incidence of liver injury,logistic regression was used to analyze the risk factors of liver injury.Result1.The number of visits were 89 patients in 2010,there were 648 patients in 2015,the number of patients in 2015 was 7.28 times that of 2010.Patients between the ages of 18 to 78 years of age,30 to 50 year old young men accounted for 44.32%,the proportion of men and women near 5.0:1;Route of infection : Heterosexual transmission was found in 961 cases(50.10%),Homosexual transmission was 804 cases(41.92%),5 cases were transfusion(0.26%),21 cases of intravenous drug abuse(1.09%),,Other and unknown 127 cases(6.62%).2.Among the 1918 cases of adult patients,the rate of HBV infection was 13.94%,the rate of HCV infection was 9.02%,and the rate of HBV/HCV infection was 1.25%.3.Among the 1918 cases of adult patients,liver function damage in 152 cases,the incidence was 7.92%,83.55% for grade 1-2 liver injury,16.45% for 3-4 liver injury.17.04% occurred in the NVP treatment group;5.98% occurred in group EFV;46.05% had HBV infection;12.50% had HCV infection,7.24% combined with HBV/HCV infection.4.The median time of liver function damage was 52 days,the main clinical manifestations of liver function injury were nausea and vomiting,appetite changes,abdominal pain.12.50% patients did not have any clinical manifestations.5.Logistics univariate regression analysis showed that age,HBs Ag+,HCV-Ab+,CD4+T lymphocyte count,the basic level of ALT,AST,higher viral load,drug group contaiming NVP,antituberculous therapy were all important risk factors.Multivariate analysis showed that age,HBs Ag+,HCVAb+,the basic level of ALT and drug group containing NVP were its independent risk factors.Conclusion1.The number of adult HIV/AIDS patients increased year by year during 2010-2015 years,from 30 to 50 year old young male dominated,age has a younger age trend;the route of infection to sexual transmission,rapid growth in the proportion of homosexual transmission,attention should be paid to.2.There is a risk of liver injury during HARRT treatment,with grade 1-2 liver injury.Age,HBsAg+,HCV-Ab+,the basic level of ALT and drug group containing NVP were independent risk factors for liver injury during HARRT treatment.3.The median duration of liver function damage during HARRT treatment was 52 days,the main manifestation was the change of liver enzyme index,Some patients did not have any clinical manifestations,should strengthen the monitoring of adverse drug reactions.
