ObjectivesIn this study,the hypertrophic scar model of rabbit ear was established by using the method of full-layer excision of the skin chondroderma.The scar thickness in the hypertrophic scar model of rabbit ear was observed by needle knife lysis and compared with the blank group.The changes of scar proliferation index and matrix metalloproteinase-1 activity were used to verify the efficacy of needle knife lysis in the treatment of hypertrophic scar,and to explore the mechanism of needle knife lysis in the treatment of hypertrophic scar.To provide experimental basis for the treatment of this disease by needle knife lysis.MethodsUsing the New Zealand White Rabbits as the experimental rabbit,according to the method of making hypertrophic scar model such as Moris and Li Huanyuan,to fabricate a square full-thickness skin and chondroid defect with a side length of 1 cm on the ventral side of the rabbit's ear,5 wounds per side of rabbit ear,a total of 100 wounds,then rabbit ear hypertrophic scar model was established.One lateral ear was randomly assigned to the acupuncture treatment group.The contralateral side was set as the scar control group.After three weeks of modeling,the treatment group was treated with Acupotomy.There was no intervention in the control group.To collecte the scar tissue specimens of treatment group and control group of 1 week?2 weeks?3 weeks?4 weeks and 5 weeks,cuting the specimen into three parts on average,cryopreservation of a specimen after rapid freezing of liquid nitrogen in-80 degrees refrigerator,After the specimen collection was completed,the homogenate of the specimen was dialyzed,using enzyme linked immunosorbent assay(Elisa)to determine the activity of matrix metalloproteinase.Preservating of neutral formalin in a specimen,The other specimen was also preserved by neutral formalin,then determing the surface density of collagen by masson staining.Results1.The scar was observed with naked eyes before treatment,1 week,2 weeks,3 weeks,4 weeks and 5 weeks.It was found that the scar bulge in the needle knife group gradually decreased,the height decreased,the range narrowed,and the scar color became light.The softness and elasticity of the model group increased,but no obvious naked eye changes were observed in the model group at the same time.2.The data of the same time between the acupuncture group and the model group at the 1st to 5th week after treatment were compared,and the difference was statistically significant(P < 0.01).The index of scar hyperplasia in acupuncture group decreased gradually with time,and the index of scar hyperplasia decreased from 3.653 before treatment to 1.659 at the 5th week.The difference was significant(P<0.01).The scar index of model group fluctuated slightly before treatment and 5 weeks after treatment.The index of scar hyperplasia changed from 3.547 before treatment to 3.699 after treatment.There was no significant difference in scar thickness before and after intervention in model group(P>0.05).3.The expression of matrix metalloproteinase 1 in the needle knife group showed an increasing trend after treatment,and the difference was significant(P<0.01)before and after the fifth week of treatment.The expression of mmp-1 in the model group was also on the rise,and the difference was statistically significant(P<0.01)before treatment and at the fifth week after treatment.There was no significant change in the expression of MMP-1 before treatment and 5 weeks after treatment in the blank group(P>0.05).There was no significant difference between the acupuncture knife group and the model group before treatment(P>0.05),but the difference was significant at the same time after treatment(P<0.05).The difference between the needle knife group and the blank group at the same time was statistically significant(P<0.01).Conclusions:1.Acupuncture can promote the local softening and flattening of hypertrophic scar in rabbit ear,regulate the thickness of hypertrophic scar,the index of scar proliferation and the activity of matrix metalloproteinase-1 in rabbit ear.2.After the treatment,the activity of matrix metallopro teinase 1 and the degradation of fibroblasts in hypertrophic scar of rabbit ear were increased.The mechanism may be that the local mechanical balance of hypertrophic scar could be restored by acupuncture knife therapy.The local microcirculation of scar was improved,the activity of matrix metalloproteinase-1 was increased,and fibroblast degradation was promoted in hyperplastic model tissue,which made the scar local softening and flattening.
