Objectives:To investigate the clinical value and effect of percutaneous radiofrequency ablation(Percutaneous Radiofrequency Ablation,PRFA)in patients with synchronous hepatic metastases of colorectal cancer,97 cases were retrospectively analyzed.Prognostic factors of the progression free survival and overall survival after PRFA were also explored.Methods:97 colorectal cancer patients with synchronous hepatic metastasis and primary tumor resected were selected in Sir Run Run Shaw Hospital from 2012.6 to 2016.6.The median age was 62 years old(31-85 years),65 males and 32 female;65 cases of patients received standard chemotherapy,32 cases with standard chemotherapy combined with cetuximab or bevacizumab;20 cases of patients without RAS gene test,50 cases were RAS negative,27 cases had RAS mutations;58 cases were left colon cancer,39 cases in the right part.All patients were divided into 2 groups based on whether PRFA were conducted.50 patients in PRFA group,47 patients in control group.Patients in PRFA group,32 cases with the maximum lesion of tumor less than 3cm,while,38 cases in control group.According to RECIST criteria,MRI,CT and CEUS scan were used to evaluate the clinical effect.The follow-up period ranged from 14 months to 62 months.The deadline of this study was in August 2017.Results:Till the last follow-up,29 case were still alive,20 died and 1 lost in RFA group.Only 1 survived in control group.In the PRFA group,6 cases without relapse,30 cases had hepatic recurrence,8 cases with lung metastasis.Median progression free survival(mPFS)and overall survival(mOS)were 13.92 and 32.86 months in RFA group,compared to 6.60 and 16.26 months in control group(p=0.000).In RFA group,the mOS with small lesions(?3cm)was 37.28 months,while 25 months in the others,p=0.001;the mOS with less tumors(?3)was 36.32 months,while 21.92 months in the left,p=0.000.No RFA related death occurred.Conclusions:The percutaneous radiofrequency ablation is an important method for colorectal cancer with hepatic metastasis.It has explicit clinical efficacy and may prolong PFS and OS,especially for small lesions and oligo-metastasis.
