Background: Lower extremity varicose veins is a common disease of vascular surgery,frequently-occurring disease,traditional treatments include palliative care,injection-compression therapy,surgery,palliative therapy is mainly applied to pregnant women,the body condition is poor,early varicose veins and patients who are intolerable surgery.Injection-compression therapy is mainly in the lesion after intravenous injection of hardener pressure dressing,making the vein wall inflammatory reaction occlusion occlusion.Currently,the treatment of varicose veins in the lower extremities is mainly based on surgery.Conventional surgery-high saphenous vein ligation and exfoliation is the gold standard for the treatment of varicose veins of the lower extremities.However,there are many defects such as incision,trauma,slow recovery and scarring.With the acceleration of the aging process in our country and people's increasing demands on the beauty,minimally invasive treatment of varicose veins of lower extremities is also increasingly respected by clinical patients.Endoscopic subfascial traffic venous disconnection,intravenous intracavitary laser closure,translucent direct-viewing rotational atherectomy,intravenous intracavitary radiofrequency closure,and coagulation are the minimally invasive treatment of varicose veins of the lower extremities,Although such surgical treatment of varicose veins have a certain effect,but because of curative effect can not replace traditional surgery as a standard surgery.With the further development of minimally invasive surgery,foam sclerotherapy is one of the main surgical methods for the treatment of varicose veins due to its unique physical characteristics,economy,safety and simplicity.This study will be compared with the traditional surgical treatment of varicose veins of lower extremity,to provide a reference for clinical treatment.Objective: To compare the clinical curative effect of foam sclerotherapy combined with high ligation of saphenous vein and traditional surgical operation(high saphenous vein ligation and exfoliation)to treat varicose veins of lower extremity,and to provide reference for clinical treatment of varicose veins of lower extremity.Method: 60 patients with varicose veins of lower extremities admitted from December 2014 to December 2016 in Jiuquan People's Hospital were enrolled in this study,according to different surgical methods are divided into two groups: the observation group of 32 patients and the control group of 28 patients.The observation group was treated with foam sclerosis combined with high lumbar saphenous vein ligation,and the control group was treated with high ligation and exfoliation of saphenous vein.(1)The operation time,the amount of bleeding,the length of hospital stay and the cost of hospitalization were compared between the two groups;(2)Visual analogue scale(VAS)was used to measure the postoperative pain scores of 1H,12 h,24h and 72 h,and the pain taking drugs were recorded after the operation;(3)The VASS(venous clinical severity score)scores of two groups of patients before operation and 3 months after operation were counted;(4)The CIVIQ(chronic venous insuficiency questionnaire)scores of two groups of patients before operation and 3 months after operation were counted;(5)The saphenous vein occlusion rate was measured by Doppler ultrasonography in two groups at 3 months and 6 months after operation;(6)The postoperative complications of the two groups were followed up,Including superficial thrombophlebitis,ecchymosis induration,residual veins,deep vein thrombosis,saphenous nerve injury and subcutaneous fat liquefaction infection;(7)Satisfaction degree of cosmetic after two groups of patients was counted.Result:(1)The observation group and the control group,both the operation time and postoperative hospitalization time were significantly shorter,while blood loss and hospitalization costs were significantly lower than the control group,with a statistically significant difference(P <0.05);(2)observation The VAS scores at 1h,12 h,24h and 72 h after operation were significantly lower than those in the control group(P <0.05).The analgesic use rate in the observation group was 3.13%,which was significantly lower than that of the control group(21.43)(P <0.05).(3)The VCSS scores of the two groups were significantly decreased at 3 months after operation compared with that before operation(P <0.05),but there was no significant difference between the two groups > 0.05).(4)The CIVIQ scores of the two groups were significantly increased at 3 months after operation compared with that before operation(P <0.05),and the difference between the two groups was statistically significant(P <0.05);(5)observation group 3 months and 6 months after the saphenous vein occlusion rate was 96.88% and 93.75%,respectively,the control group were 100% and 95.24%,no significant difference between groups(P> 0.05);(6)observed The postoperative incidence of subcutaneous ecchymosis was 9.38%,which was significantly lower than that of the control group 21.43%,a significant difference(P <0.05);(7)followed up for 6 months to observe their beauty group after satisfaction 96.88% to 71.43% in the control group,statistically significant(P <0.05).Conclusion: Foam sclerotherapy combined with lumbar saphenous vein ligation and traditional surgical treatment of lower extremity varicose veins are effective,but the former has a small incision,trauma,fewer complications,low cost,postoperative pain and other advantages,can replace traditional surgery.
