Objective: To observe the effect of intestinal cleaning self-assessment on colonoscopy preparation process,colonoscopy operation process and examination results,and the clinical application value of this method in intestinal cleaning in QingHai area of plateau.Methods:1.From May 2017 to November,the outpatients who visitted the Digestive Endoscopy Center of the affiliated Hospital of QingHai University for the first time to undergo colonoscopy,and was able to complete the preparation and examination of the intestine.579 outpatients who met the criteria of admission and selection were randomly divided into self-assessment group(294 cases)and control group(285 cases).2.The self-evaluation group cleaned the intestines according to the intestinal cleaning and self-assessment method,and the control group cleaned the intestines according to the routine method of the endoscopy center.3.The general data of two groups of bowel preparation were collected,and the information filled by intestinal preparation patients during intestinal cleansing process and data from doctors during colonoscopy verification phase were collected for statistical analysis.Results:1.The two groups have consistency(p> 0.05)in age,sex,nationality,education level,risk factors of colon cancer,time interval between symptoms to colonoscopy,and knowledge about disease and intestinal cleanliness.2.In the process of intestinal cleaning,the time of taking medicine in selfassessment group was less than that in control group(p <0.05);The amount of each time additional water and total additional water taken after two times of medication was more than that of the control group,and the number of defecation after take medicine for the first time was more than that of the control group;The number of defecation times after second times of taking medicine and the total defecation number of two times after taking the medicine,the number of defecation in the selfevaluation group and the control group was equal(p > 0.05).3.In the process of intestinal validation,there was no difference between the last defecation time between the two groups and the time interval of the examination(P > 0.05);However,in the score of intestinal cleanliness,the detection rate of polyps and the success rate of inserting colonoscopy,the self-assessment group was more than or higher than the control group(p <0.05);In terms of the time of entering the colonoscopy and the extra operation time,the control group used more time than the self assessment group;there was no difference between the two groups in the time of returning the colonoscopy and the failure rate of bowel preparation.4.The self-assessment group was evaluating the role of "intestinal cleaning selfassessment " in the bowel preparation process: 95% of the bowel preparation patients thought there were different degrees of help.Conclusions:1.In the process of bowel preparation,the intestinal self-assessment method can improve the drug dependence of the bowel preparation,increase the extra water intake and the frequency of defecation after the first medication;but there was no influence on the number of defecation and the number of total defecation after second doses of medicine.2.The cleanliness of the intestine prepared by the intestinal cleaning selfassessment method was improved,the method can shorten the time of entering the colonoscopy and the extra operation time in operating process,and increase the success rate of inserting colonoscopy and the detection rate of polyps;but it has no effect on the time of withdrawal and the failure rate of bowel preparation.3.The intestinal cleaning self-assessment method can help 95% of the intestinal preparations in different degrees.4.This method increases the self-assessment method of intestinal preparation process and has certain economic benefits and social value,which is worthy of reference in clinical practice.
