Recurrence Assessment Value Of Acute Exacerbation Of Chronic Obstructive Pulmonary Disease Patients By Plasma Obestatin And Ghrelin Levels

Abstract/Summary:
Objective To study the recurrence assessment value of acute exacerbation of chronic obstructive pulmonary disease(AECOPD)patients by plasma obestatin and ghrelin levels within 1 year.Methods 120 AECOPD patients of grade C and D were enrolled in terms of inclusion criteria from Global Initiative for Chronic Obstructive Lung Disease(GOLD)guideline,which were divided into group A(low weight,BMI≤18.5kg/m2),group B(normal weight,18.5 kg/m2<BMI≤24.9 kg/m2)and group C(overweight,BMI>24.9kg/m2).Pulmonary function testing(PFT),6 minute walking test(6MWT),COPD assessment test(CAT),plasma obestatin and ghrelin levels,routine blood test,high sensitivity C-reaction protein(hs-CRP),plasma IL-6,plasma IL-8,NT-Pro BNP,arterial blood gas,acute exacerbation frequency and mortality were followed up and recorded in each group at admission,at discharge,6 months after discharge and 12 months after discharge.Results 1.The numbers of acute exacerbations in group A and C were significantly higher than those in group B(P < 0.05);And the mortality in group A was significantly higher than that in group B and C(P<0.05).However,there was no statistically significance that the mortality of group C was higher than that of group B.2.At admission and after one year follow-up,BMI,plasma ghrelin and obestatin levels were statistically significant among three groups(P<0.05).With BMI increasing,plasma ghrelin concentration increased,and plasma obestatin concentration decreased;And at admission plasma obestatin and ghrelin levels were higher than those at discharge and after one year follow-up among three groups(P<0.05).3.Plasma IL-6 and IL-8 levels and hs-CRP were statistically significant among three groups at admission(P<0.05),but leukocytes were not statistically significant among three groups(P>0.05);The concentritons of plasma IL-6 and IL-8 and hs-CRP were all group A>group C>group B.After one year fowllow-up,plasma IL-6 and IL-8 levels,hs-CRP and leukocytes were not statistically significant among three groups(P>0.05);At admission plasma IL-6 and IL-8 levels,hs-CRP and leukocytes were higher than those at discharge and one year follow-up(P<0.05).4.The plasma obestatin and ghrelin levels were positively correlated with hs-CRP,plasma IL-6 and IL-8 levels(P <0.05),but they had no significant correlation with leukocytes.The plasma obestatin level was negatively correlated with BMI and the plasma ghrelin level was positively correlated with BMI at different time points(P <0.05).5.Others:At admission and after one year follow-up,NT-pro BNPs in group C were higher than those in group A and B(P<0.05).At admission,Pa O2 of group A was lower than that of group B and group C(P<0.05);And after one year follow-up,p H value and Pa CO2 of group A were statistically significant compared with those of group B and group C(P<0.05).At admission and after one year follow-up,FEV1(L),6WMT and CAT scores were not statistically significant among three groups(P>0.05).ConclusionBMI,plasma obestatin and ghrelin levels may be associated with AECOPD;The plasma obestatin and ghrelin levels in COPD patients may be used as new biomarkers to predict acute exacerbations.
