Objective:To analyze the distribution of body mass index in patients with Acute exacerbation of Chronic obstructive pulmonary disease and the effects of different body mass indexes on pulmonary function and inflammation in patients with chronic obstructive pulmonary disease,and to investigate the correlation between biochemical indexes such as blood lipids and proteins and pulmonary function and inflammatory factors in patients with chronic obstructive pulmonary disease.Methods:85 patients with acute exacerbation of chronic obstructive pulmonary disease were collected from three wards of respiratory medicine department of the second affiliated hospital of kunming medical university from september 2016 to september 2019.Retrospective analysis was used to analyze and compare its general clinical data,biochemistry,blood lipids,acute infection,lung function and so on.Results:1.This study included 85 AECOPD patients,which were divided into three groups according to body mass index(BMI).Among them,27 cases were lean group,accounting for 32%,3 8 cases were normal weight group,accounting for 44%,and 20 cases were overweight group,accounting for 24%2.Gender,age and smoking years were not statistically significant between the three groups(P>0.05).There was statistical significance in comparing hyperlipidemia and GOLD grading among the three groups(P<0.05).The proportion of hyperlipidemia:overweight group greater than normal weight group greater than lean group,and the difference between overweight group and lean group was statistically significant,and the difference between overweight group and normal weight group was not statistically significant.In the GOLD ? and ? groups,three groups accounted for:overweight group greater than normal weight group greater than lean group,In the GOLD ? and ? groups,three groups accounted for:overweight group less than normal weight group less than lean group,the difference between two groups was not statistically significant(P>0.05).3.There was no significant difference in prealbumin and HB?TC?LDL-C between the three groups(P>0.05),the total protein,albumin and TG?HDL-C were statistically significant(P<0.05).The total protein,albumin and TG in overweight group were significantly higher than those in lean group(P<0.05).Compared with normal weight group,the total protein and albumin in overweight group increased significantly(P<0.05).The TG of overweight group was higher than that of normal weight group,but the difference was not statistically significant(P>0.05).The total protein,albumin and TG in normal weight group were higher than those in lean group,but the difference was not statistically significant(P>0.05).The HDL-C of overweight group was significantly lower than that of lean group(P<0.05).The HDL-C of overweight group was significantly lower than that of normal weight group(P<0.05).The HDL-C of normal weight group was lower than that of lean group,but the difference was not statistically significant(P>0.05).4.The comparison between the three groups FEV1/FVC?PCT?IL-6?hs-CRP not statistically significant(P>0.05),and the comparison between the three groups FEV1%pred statistically significant(P>0.05).The FEV1%pred of overweight group was significantly higher than that of lean group(P<0.05);the FEV1%pred of overweight group was higher than that of normal weight group,the difference was not statistically significant(P>0.05),and the FEV1%pred of normal weight group was higher than lean group,and the difference was statistically significant(P<0.05).5.BMI is positively correlated with FEV1%pred?FEV1/FVC.HDL-C is negatively correlated with FEV1/FVC and negatively correlated with hs-CRP.LDL-C is negatively correlated with PCT.HB is negatively correlated with hs-CRP.total albumin was positively correlated with FEV1/FVC.anterior albumin was negatively correlated with PCT?IL-6?hs-CRP.6.GOLD grading was positively correlated with smoking and negatively correlated with BMI?prealbumin.7.Results of multiple ordered regression analysis:BMI is the influencing factor of GOLD grading.Conclusion:1.The higher the BMI in AECOPD,the more the proportion of hyperlipidemia,the more the GOLD grades are distributed in the ? and ? grades,the less the distribution is in the ? and ? grades,and the higher the total serum protein,albumin and TG?FEV1%pred,the lower the HDL-C.2.The relationship between nutritional index and lung function in AECOPD patients:BMI and FEV1%pred?FEV1/FVC were positively correlated,total protein and FEV1/FVC were positively correlated,and FEV1/FVC were negatively correlated.3.The relationship between nutritional index and inflammatory index in AECOPD patients:prealbumin and PCT?TL-6?hs-CRP were negatively correlated,HB and hs-CRP were negatively correlated,LDL-C and PCT were negatively correlated,HDL-C and hs-CRP were negatively correlated.4.BMI is the influencing factor of gold grading severity,and the BMI is negatively correlated with GOLD grading,that is,the lower the BMI,the higher the grading.
