Development, Quantitative Grading And Evaluation Of The Diagnostic Scale Of Coronary Heart Disease And Heat Syndrome

According to the latest cardiovascular report,the prevalence of cardiovascular disease(CVD)is still on the rise,with about 11 million people suffering from the disease,the mortality rate is increasing year by year,and the rural population is stronger than the urban population.Phlegm-heat syndrome is a common TCM syndrome of coronary heart disease,but the implementation of primary health service diagnosis and treatment standards is still uneven[1],with the implementation of the three-level diagnosis and treatment system,primary medical units of coronary heart disease patients will gradually increase,the implementation of standardized diagnosis and treatment of coronary heart disease,the establishment of a scientific coronary heart disease diagnosis scale is conducive to accurately determine the type of TCM syndrome of coronary heart disease,to provide effective means to solve the pain of patients with coronary heart disease in a timely manner.Objective:Based on the theory of traditional Chinese medicine(TCM),a diagnostic scale for patients with phlegm-heat syndrome of coronary heart disease(CHD)is developed,which provides a standardized evaluation tool for TCM diagnosis of CHD with phlegm-heat syndrome.At this stage,on the basis of the previous study of TCM four diagnostic information table of phlegm-heat syndrome of coronary heart disease,the scale structure framework is weighted,the scale diagnostic threshold is established,the diagnostic scale of phlegm-heat syndrome of coronary heart disease is established,and the scale is graded.So that the scale can be more scientific and objective,lay the foundation for its evaluation and clinical application,and contribute to improving the level of community medical and health services.Methods:292 patients who were included in the database were standardized,based on the four diagnostic information tables of phlegm-heat syndrome,SPSS 23.0 was used to weight the items of the scale,logistic regression model was used to determine the contribution of each dimension of the scale,roc curve was drawn,the diagnostic threshold was determined,and the diagnostic scale of concerned phlegm-heat syndrome was established.According to Seattle scale and cardiac function classification of authority scale in coronary heart disease field,the evaluation value of this scale was tested by consistency analysis with this scale.The diagnostic scale of phlegm-heat syndrome of coronary heart disease was classified quantitatively by percentage method and discriminant analysis method,and the diagnostic scale of phlegm-heat syndrome of coronary heart disease was established.Results:this study established the diagnostic scale of phlegm-heat syndrome in coronary heart disease.The diagnostic accuracy was 91.8%,sensitivity was 92.1 and specificity was 90%,which indicated that the scale had good diagnostic function.The frequency percentage method is used for simple classification of the current sample,and the result shows that the patient with mild phlegm-heat syndrome of coronary heart disease is obtained when the score of the phlegm-heat syndrome scale is between 13 and 19,the patient with moderate phlegm-heat syndrome of coronary heart disease is obtained when the score of the phlegm-heat syndrome scale is between 20 and 28,and the patient is judged to be a patient with severe phlegm-heat syndrome of coronary heart disease when the score of the phlegm-heat syndrome scale is more than 28.The correct rates of self-test and interaction test are 92.8%and 88.7%respectively.Conclusion:This study established the diagnostic scale of phlegm-heat syndrome in coronary heart disease.The diagnostic accuracy was 91.8%,the sensitivity was 92.1 and the specificity was 90%.The frequency percentage method is used for simple classification of the current sample,and the result shows that the patient with mild phlegm-heat syndrome of coronary heart disease is obtained when the score of the phlegm-heat syndrome scale is between 13 and 19,the patient with moderate phlegm-heat syndrome of coronary heart disease is obtained when the score of the phlegm-heat syndrome scale is between 20 and 28,and the patient is judged to be a patient with severe phlegm-heat syndrome of coronary heart disease when the score of the phlegm-heat syndrome scale is more than 28.In addition,fisher discriminant equation is established to provide an effective tool for further accurate classification.
