Based On Bayesian Network Technology, The Study Of TCM Syndromes Of Anxiety And Depression Comorbidity

Objective:This article uses Bayesian network method to analyze the TCM syndrome data of patients with anxiety and depression comorbidities collected,extract the main symptom factors of the disease,and further use Logistic regression analysis to explore the TCM syndrome classification of anxiety and depression comorbidity.And syndrome characteristics to guide clinical diagnosis and treatment.Methods:In this study,a cross-sectional study was conducted,and two-year case collection was conducted in four hospitals,Beijing Anding Hospital,Beijing Huilongguan Hospital,Beijing University of Traditional Chinese Medicine Third Affiliated Hospital,and Lianyungang City Hospital.Self-rating anxiety scales and self-rating depression scales were used to screen suspicious patients,and patients with comorbid anxiety,depression,and depression were further completed.The data of medical records were organized,Bayes net technology and Logistic regression analysis were used to mine the data,and combined with traditional Chinese medicine theory and expert experience,the traditional Chinese medicine syndromes of anxiety and depression were investigated.Result:(1)Analysis of the frequency and symptoms of 523 CAD patients with symptoms and physical signs.The top 25 were worried,nervous,upset and irritated,difficulty falling asleep,fatigue,dreaminess,and uninteresting interest.Dizziness,suspicious,unrequited mind,fear,apathy,good for the rest,forgetfulness,dry mouth,sorrow,sentimentality,timidity,back pain,early awakening,head weight,chest tightness,shortness of breath,dry eyes,anorexia,bloating,fear Cold and cold,feet and sour,from the overall data,it basically reflects the common symptoms of anxiety and depression.(2)This study analyzed the symptoms and signs of 523 CAD patients through Bayesian network technology and extracted 17 syndrome elements:liver qi stagnation,liver wang wang,hepatic blood deficiency,heart fire wang,heart qi deficiency,heart and blood deficiency,heart yin deficiency,Spleen qi deficiency,spleen yang deficiency,kidney qi deficiency,kidney yang deficiency,kidney yin deficiency,kidney essence deficiency,gallbladder qi deficiency,gallbladder ignorance,blood stasis,and dampness were used to establish a Bayesian network model again for the above 17 syndrome elements.We further extracted seven types of CAD syndromes,namely,deficiency of heart and gallbladder,deficiency of cardio-renal syndrome,syndrome of liver qi stagnation,deficiency of kidney and liver qi,deficiency of spleen and kidney yang,deficiency of heart and spleen,and deficiency of kidney and liver qi.Among them,the proportion of heart-biliary-deficiency-deficiency syndrome was the highest,which was 40.0%,that of liver-qi stagnation-fire syndrome was 18.9%,that of heart-kidney-in-aid was 18.4%,that of kidney-deficiency and liver-depression was 14.0%,and that of heart and spleen deficiency was 6.9.%,Liver depression and spleen deficiency accounted for 1.9%,spleen and kidney yang deficiency accounted for 1.3%.(3)Logistic regression analysis was used to analyze the symptoms of the four major syndromes and their symptoms,and the symptom distribution and primary and secondary symptoms of CAD syndromes were summarized.The main symptoms of heart-biliary-qi deficiency are:timid,frightened,palpitation,spontaneous perspiration;secondary symptoms:Shenpi fatigue,suspicious.Heart and kidney do not pay the main symptoms of syndrome:Upset irritability,tinnitus,dreams,five upset hot;Second symptoms:palpitations,hot flashes,night sweats,facial hot.The main symptoms of liver qi stagnation syndrome:upset irritability,sad sentimentality,good for the rest,dry mouth,more dreams;secondary symptoms:flank pain,breast pain,dizziness,headache,dry eyes,constipation,menstruation is not Tone.The main symptoms of kidney deficiency and liver depression syndrome:fear,tinnitus,sorrow,sentimentality,good fortune,back pain,and more dreams;secondary symptoms:flank pain,breast pain,dizziness,sore knees,Shenpi fatigue,irregular menstruation Premature ejaculation.The main symptoms of spleen-kidney yang deficiency:fear,fatigue,bloating,low libido,urinary frequency,aversion to cold and coldness;secondary symptoms:detachment,forgetfulness,anorexia,belching,diarrhea,nausea,premature ejaculation,irregular menstruation,impotence.The main symptoms of heart and spleen deficiency:palpitations,suspiciousness,philosophical thoughts,forgetfulness,sadness,cry,anomaly,dreaminess,early awakening;secondary symptoms:always crying,fatigue,bloating,belching,nausea Spit,diarrhea.The main symptoms of liver depression and spleen deficiency syndrome:good for the rest,sad feelings,Shenpi fatigue,bloating,diarrhea;secondary symptoms:dreams,breast tenderness,thoughtlessness,forgetfulness,rib pain,anorexia,belching,nausea Want to vomit.(4)In accordance with the data mining method of association group in the early period of this research group,five TCM syndromes[19]with anxiety and depression comorbidity were summarized,namely liver fire stasis syndrome,kidney deficiency and liver depression syndrome,heart and gallbladder qi deficiency syndrome,liver depression and spleen deficiency syndrome,spleen and kidney yang deficiency.Conclusion:(1)In this study,Bayesian network technology and Logistic regression analysis methods were used to explore the distribution of TCM syndromes in comorbid anxiety and depression.Summarize 7 TCM syndromes of CAD:heart and gallbladder Qi deficiency syndrome,heart and kidney failure syndrome,liver qi stagnation syndrome,kidney deficiency and liver depression syndrome,spleen and kidney yang deficiency syndrome,heart and spleen deficiency syndrome,liver depression and spleen deficiency syndrome,and The primary and secondary symptoms of each syndrome.It was found that the etiological elements of the disease are closely related to the heart,liver and kidneys,and also related to the gallbladder and spleen;the disease factors are closely related to qi depression,fiery fever,and qi deficiency.(2)Compared with previous data mining methods of this research group,there are 5 syndrome overlaps in TCM syndromes of anxiety-depression comorbidity,namely liver-fire stasis syndrome,kidney deficiency and liver stagnation syndrome,and heart-bile-qi deficiency syndrome,Spleen Deficiency Liver Depression Syndrom,Spleen Kidney Yang Deficiency Syndrome.(3)Bayesian network,association rules and Logistic regression are all effective methods for extracting the elements of TCM syndromes.
