Spastic hemiplegia cerebral palsy is one of the most common types of cerebral palsy.This type of cerebral palsy due to one side limb involved in hemiplegic gait,It has the compensatory poor neural during coordination walking exercise,has short stride length,stride frequency and low stability characteristics.At the same time,there are many problems such as the long time of supporting time and the short swingin in the uninjured side.On the basis of Bobath therapy,Ye Cangfu,a therapist in Taiwan,put forward the method of uninjured side unilateral lower limb loading to intervene the pathological gait of hemiplegic children with cerebral palsy.Unilateral unilateral lower limb loading is a kind of abnormal weight intervention.At present the study about unilateral unilateral lower limb loading walking is still blank in our country,and there are a lot of foreign related research,and some research support has significance for the treatment of unilateral unilateral lower limb loading for stroke compensatory gait.The compensatory gait of cerebral palsy is highly similar to that of stroke.Therefore,this experiment intends to determine the effects of unilateral lower limb weight training on walking ability of spastic hemiplegia cerebral palsy,determine the bilateral lower limb kinematics parametersands change and majority lower muscles sEMG changes.In order to understand the changes of walking parameters in the body and limbs of the contralateral side of the body.To investigate the effect of exercise control system on unilateral lower limb loading walking.To further understand the effect of the unilateral lower limb loading on children with hemiplegic cerebral palsy,and to explore the effectiveness of the treatment of the contralateral side.Research object and method:Methods 12 children with hemiplegic spastic cerebral palsy were selected,and the subjects were subjected to unilateral lower limb loading exercise training for 20 minutes walking after the first time take the sandbag on the foot of the lower limb at the ankle joint.Before and after intervention,and immediately after intervention,the bilateral lower limb kinematics parameters,time and space parameters,sEMG data were collected.Using statistical methods analysis the date.Research results:1 The intervention time of both the uninjured side and the contralateral side was significantly reduced compared with that before the intervention(P<0.05),and the contralateral side of the lower limb was significantly increased after intervention.2 Affected side step size increased immediately after intervention and after intervention,but no significant.3 There was no significant change in the range of knee and ankle joint immediately after intervention and after intervention4 The average amplitude of the contralateral tibialis anterior muscle,the femoral head,the gastrocnemius muscle and the gastrocnemius muscle during the intervention period immediately after intervention was significantly higher than that before the intervention(P<0.05).The mean amplitude of the anterior tibial muscle and the tibialis anterior muscle of the affected side was significantly lower than that of the intervention(P<0.05).The average amplitude of the contralateral tibialis anterior muscle was significantly higher than that before intervention(P<0.05).6 The average amplitude of the contralateral gastrocnemius muscle support was significantly higher than that before intervention(P<0.05).The mean amplitude of the contralateral gastrocnemius muscle support was significantly lower than that of intervention(P<0.05).The mean amplitude of the contralateral gastrocnemius swing phase was significantly lower than that of intervention(P<0.05).The average amplitude of the contralateral gastrocnemius muscle was significantly increased after intervention(P<0.05).7 The average amplitude of the intervention of the biceps femoris muscle during the swing phase of the two patients was significantly increased compared with that before intervention(P<0.05).The average amplitude of the two sides of the femoral head and the average amplitude of the swing period were significantly lower than that of intervention(P<0.05).The average amplitude of the two sides of the biceps femoris muscle during the support phase and swing phase was significantly higher than that before intervention(P<0.05).8 The average amplitude of the rectus femoris muscle swing period wassignificantly higher than that of intervention(P<0.05).The average amplitude of the contralateral rectus femoris muscle was significantly increased after intervention(P<0.05).The average amplitude of the contralateral rectus femoris swing period was significantly increased compared with that before intervention(P<0.05).The mean amplitude of the contralateral rectus femoris muscle was significantly reduced compared with that before intervention(P<0.05).Research conclusion:1 Immediate intervention both lower extremities shaking peroid and support period both significantly increased.After intervention both of them have a tendency to reduce.The uninjured sides support period reduce more.The affected side keep increasing,indicate that after intervention the walking stability of the affected side increased.The proportion of swing time in both lower limb walking cycles increased immediately after intervention,suggesting that gait ability was improved.2 The unilateral lower limb loading intervention did not significantly improve the range of motion of the affected knee and ankle.The unilateral lower limb loading intervention immediately broke the original stable gait pattern,but after the intervention has established a stable gait.3 The sEMG data of each muscle of lower limbs showed that the unilateral lower limb loading of the lower limbs could increase the activation degree of the seven muscles except the lateral rectus femoris muscle,but the level of pre intervention was quickly restored after intervention.However,the lateral rectus femoris muscle remained low activation level,suggesting that the activation of the rectus femoris muscle was increased and the gait stability was improved.To sum up,the walking ability of the subjects was improved immediately after the intervention of the contralateral weight intervention,but the subjects recovered quickly after 20 minutes of intervention.The results showed that unilateral unilateral lower limb loading intervention can improve the walking ability of the subjects,but the body will quickly adapt to change.
