Objective:To understand the level of environmental pollution in public places and the status of legionella pneumonia infection of practitioners in public places;analyze the exposure relationship between the legionella pneumonia infection status of practitioners in public places and the environmental impact factors;and provide the theoretical foundation for controlling the prevalence of infectious diseases in public places.Methods:Seven public places in Wuxi and Changzhou were chosen and the common pathogen microorganism in central air conditioning ventilation system,legionella pneumonia,was selected as the study subject.Analyze the level of environmental exposure level of legionella pneumonia pollution which possibly existed in the cooling water,tap water,shower water,landscape water,flowers soil,and air supply of central air-conditioning system in typical public places,such as hotel/restaurant and shopping mall/supermarket.Choose practitioners exposed to the central air conditioning environment in a long term in above seven public places.Investigate the legionella pneumophila infection status of practitioners in public places by ELISA test of serum antibody of legionella pneumonia.For the positive rate or constituent ratio of different groups,analyze whether the difference among different groups has statistical significance by adopting the chi-square test;adopt the multiple Logistic regression methods to analyze the impact factors of legionella pneumonia infection in the population.Results:the positive rate of the legionella pneumonia in aerosol,flower soil,shower water,landscape water,tap water,cooling water,and dust in air duct in public places are 31.43%(22/70),31.29%(10/32),40.74%(11/27),33.33%(1/3),7.41%(2/27),64.29(9/14),and 40.00%(2/5),respectively.The total positive rate difference of PCR test on legionella pneumophilia in environmental samples in different regions has statistical significance(x2=14.655,P=0.000).The positive rate of test result of serum antibody of legionella pneumonia of practitioners in public places was 35.16%(96/273),and the infection rate of legionella pneumonia of practitioners in public places of Changzhou is higher than that of Wuxi city;the infection rate of legionella pneumonia(59.65%)of the people who stay in other public places besides the work place for a while was significantly higher than that of the people who do not stay(30.49%).Central air conditioning cooling water in public places is a risk factor for the legionella pneumonia infection of practitioners(OR = 3.001).Conclusion:based on the research results,the legionella pneumonia pollution widely exist in two types of public places,the hotel/restaurant and shopping mall/supermarket in Changzhou and Wuxi,and the legionella pneumonia pollution in the two types of public places in Changzhou is higher than that in Wuxi.The positive rate of serum antibody of legionella pneumonia of practitioners in Changzhou is higher than that in Wuxi,which is consistent with the results of environmental investigation.The positive rate of serum antibody of legionella pneumonia of practitioners who stay in other public places besides the work place for a while is higher than that of practitioners who do not stay.The cooling water of central air conditioning cooling tower in public places is a risk factor for the legionella pneumonia infection of practitioners.The cooling water is one of the most important sources of legionella pneumonia infection in the population,and the effective and reasonable disinfection measures of the cooling water tower on a regular basis could efficiently reduce the risk of legionella pneumonia infection in the population.
