Objective: To observe whether 3D printing combined with conventional open reduction and internal fixation in patients with complex tibial plateau fracture can reduce the operative time and blood loss in patients with complicated tibial plateau fracture compared with those patients treated with conventional open reduction and internal fixation alone.The length of incision and the improvement of knee function after operation were used to evaluate the effect of 3D printing in the diagnosis and treatment of complex tibial plateau fractures.Methods: From January 2016 to September 2017 Guangxi University of Chinese Medicine Affiliated Ruikang Hospital of Department of orthopedics treatment of Schatzker type to complex tibial plateau fractures in patients with 31 cases of type IV above,by 2 senior deputy director of Department of orthopedics physicians and 1 radiologists read and agreed to split the type IV above were included in the study,will the use of 3D printing technology combined with open reduction and internal fixation of patients in the observation group,treated by conventional open reduction and internal fixation of the patients in the control group,16 cases in the observation group,15 cases in the control group.Patients in the observation group were fracture model using 3D printing 3D printing technology,and to evaluate the classification,surgical design,improve the simulation operation details of fracture,and then the conventional open reduction and internal fixation;patients in the control group with the conventional X-ray film,CT and 3D reconstruction imaging data classification evaluation surgical design,fracture,and conventional open reduction and internal fixation.The operative time,intraoperative blood loss,incision length,and KSS function score,scoring scale and other indicators of two groups of patients were recorded in detail in first groups,third months and sixth months after operation.Results: The patients in the control group and the experimental group were all patients with tibial plateau fracture above type ?according to Schatzker classification.There was no significant difference in age and sex between the two groups(P > 0.05).The data analysis showed that the operation time was long and the amount of blood lost during operation in the experimental group was not significant(P > 0.05).The length of incision was smaller than that of the control group(P < 0.05),the KSS score of knee joint in the experimental group was significantly improved than that in the control group at the 1st,3rd and 6th month after operation(P < 0.05),but there was no significant difference between the two groups in the grading of knee joint KSS function at the 1st,3rd and 6th month after operation(P > 0.05).Conclusion(s): In the 3D printing technology application process can effectively reduce the surgical treatment of type of type IV patients over Schatzker complex fractures of the tibial plateau preoperative time,reduce bleeding,reduce the incision range,thereby reducing surgical trauma;surgery can improve the knee function score,has good clinical application value.
