Objective:The objective of this study is to evaluate the sleep status of chronic insomnia disorder patients before and after the inhalation therapy with Taxus chinensis sustained release granules,and study the effect of inhaling Taxus chinensis sustained release granules on chronic insomnia disorder.Methods:20 patients with chronic insomnia disorder(aged 18-65 years)who were admitted to the sleep centre of Department of Neurology,the First Hospital of Jilin University from September 15 th 2016 to March 15 th 2017 were selected.In order to rule out the first night effect to the result of the experiment,20 patients were admitted to the sleep monitoring room of the hospital on the first day of the experiment.On the second day of the experiment,20 patients were in the sleep monitoring room for PSG.After the PSG monitoring,the PSQI scale and ISI scale were evaluated.On the third day and fourth day of the experiment,20 patients went home to use the Taxus chinensis sustained release granules sleeping pillow.On the fifth day of the experiment,20 patients were in the sleep monitoring room for PSG again andused the Taxus chinensis sustained release granules sleeping pillow.After PSG monitoring,the PSQI scale and ISI scale were evaluated again.During the experiment,patients should record sleep diary every day and prohibit taking sedative drugs.The study adopts a self-control method.Compared the changes of sleep latency,total sleep time and awakening time by sleep dairy before and after treatment by inhaling Taxus chinensis sustained release granules.Compared the scores of PSQI scale and ISI scale before and after treatment by inhaling Taxus chinensis sustained release granules.Compared the changes of total sleep time,sleep latency,sleep efficiency,awakening time,arousal times,microarousal index,spontaneous microarousal index,the sleep structure of PSG before and after treatment by inhaling Taxus chinensis sustained release granules.Compared the subjective sleep latency and total sleep time with objective sleep latency and total sleep time.Result:(1)sleep diary: Compared with the sleep latency before treatment [35.00(25.00~50.00)min],the sleep latency time after treatment [25.00(20.00~33.75)min] was shorten,the difference was statistically significant(p=0.003<0.01).The arousal time after treatment(39.75±31.43 min)was significantly reduced than before treatment(70.75±38.74 min),the difference was statistically significant(p=0.001<0.01).The total sleep time after treatment(382.00±58.99 min)was significantly higher than before treatment(349.00±64.39 min),the difference was statistically significant(p= 0.013< 0.01).(2)The total score of PSQI after treatment(11.60±2.98)was reduced than before treatment(13.00±2.32),the difference was statistically significant(p=0.001 < 0.01);the total score of ISI after treatment(17.00±4.85)was reduced than before treatment(18.40±4.72),the difference was statistically significant(p = 0.003< 0.01).(3)PSG results showed that the sleep latency time after treatment [3.85(0.93~7.50)min]was reduced than before treatment [7.20(2.28~24.35)min],the difference was statistically significant(p=0.008<0.01).The microarousal index after treatment [23.60(20.10~25.95)]was reduced than before treatment [27.70(18.85~35.05)],the difference was statistically significant(p=0.028 < 0.05).The spontaneous microarousal index after treatment[16.95(9.50~20.40)] was reduced than before treatment [19.80(11.85~28.98)],the difference was statistically significant(p=0.018<0.05).(4)Comparison of subjective and objective sleep evaluation results: The patients with chronic insomnia disorder overestimated the sleep latency [before treatment subjective sleep latency 35.00(25.00~50.00)min VS before treatment objective sleep latency 7.20(2.28~24.35)min,p< 0.001;after treatment subjective sleep latency 25.00(20.00~33.75)min VS after treatment objective sleep latency 3.85(0.93~7.50)min,p<0.001],and underestimated the total sleep time [before treatment subjective total sleep time 349.00±64.39 min VS before treatment objective total sleep time 386.50±75.14 min,p< 0.001;after treatment subjective total sleep time 382.00±58.99 min VS after treatment objective total sleep time 391.55±69.15 min,p=0.028<0.05].Conclusion:1.The inhalation of Taxus chinensis sustained release granules can make the sleep latency time shorter,increase the total sleep time and decrease the awakening time of subjective feeling.It can improve the sleep quality and reduce the severity of insomnia.2.The inhalation of Taxus chinensis sustained release granules can also shorten sleep latency time,decrease the microarousal index and spontaneous microarousal in PSG.Therefore inhalation of Taxus chinensis sustained release granules is effective on patients with chronic insomnia disorder.3.The subjective and objective sleep evaluation results of patients with chronic insomnia are inconsistent.Patients with chronic insomnia disorder underestimate the total sleep time and overestimate the sleep latency.
