Objective:Posterior circulation ischemia caused by atheroscleros is is an important pathogeny in vascular vertigo,which accounted for the main cause of dizziness with 60%.Studies have shown that infl ammatory reaction and can lead to atherosclerosis.As inflammatory mediator,CRP,Lp-PLA2 and sCD40 L involved in this process.In this study,we analysed CRP,Lp-PLA2 s CD40 L levels in patients with va scular vertigo to explore the correlation between these three factors a nd atherosclerotic vascular vertigo,in order to provide the basis for ea rly prevention and treatment of disease of atherosclerotic vascular ver tigo and related cerebrovascular disease.Methods:1.We selected 91 patients with vertigo,who's average age is?61.81±6.799?years,all patients were in our standard.Accordin g to the examination results of TCD,MRA and vestibular function te sts,the patients were divided into atherosclerotic vascular vertigo gro up 48 cases?include 25 males and 23 females?and non-vascular verti go group 43 cases?include 23 males and 20 females?.2.At the sam e time,we selected 40 healthy persons?include 21 males and 19 femal es?without clinical evidence of vertigo as the control group.3.All i ncluded subjects were measured plasma CRP,Lp-PLA2,sCD40 L by latex immunoturbidimetry,UPM method and enzyme-linked immunos orbent assay?ELISA?,the data were compared among groups.4.Anal ysised the correlation between plasma CRP,Lp-PLA2,s CD40 L and at herosclerotic vascular vertigo and its diagnostic value.Result:1.The level of plasma CRP in atherosclerotic vascular vertigo gr oup was?0.564±0.716 7?mg·L-1,the level in non-vascular vertigo group was?0.232±0.235 0?mg·L-1and in control group was?0.175±0.141 0?mg·L-1.The level of plasma CRP in atherosclerotic vas cular vertigo group was higher than non-vascular vertigo group and c ontrol group,the difference was statistically significant?P<0.05?.The re was no significant difference in plasma CRP levels between non-v ascular vertigo group and control group,with no statistical significanc e?P>0.05?.2.The level of plasma Lp-PLA2 in atherosclerotic vascular vertig o group was?203.166 8±86.745 28?ng·mL-1,the level in non-vascul ar vertigo group was?104.879 8±25.517 26?ng·mL-1and in control group was?98.172 2±25.974 87?ng·m L-1.The level of plasma Lp-P LA2 in atherosclerotic vascular vertigo group was higher than non-va scular vertigo group and control group,the difference was statistically significant?P<0.05?.There was no significant difference in plasma Lp-PLA2 levels between non-vascular vertigo group and control grou p,with no statistical significance?P>0.05?.3.The level of s CD40 L in atherosclerotic vascular vertigo group was?658.878 40±287.711 119?pg·m L-1,the level in non-vascular vertigo group was?329.847 28±190.829 191?pg·mL-1and in control group was?340.373 0±245.570 585?pg·mL-1.The level of sCD40 L in vascular atherosclerotic vertigo group was higher than non-vascu lar vertigo group and control group,the difference was statistically si gnificant?P<0.05?.There was no significant difference in sCD40 L le vels between non-vascular vertigo group and control group,with no s tatistical significance?P>0.05?.4.Patients with atherosclerotic vascular vertigo have a higher pre valence of hypertension and diabetes than those with non-vascular ver tigo?P<0.05?.5.Plasma Lp-PLA2 levels are positively correlated with LDL lev els in patients with atherosclerotic vascular vertigo;CRP,sCD40 L lev els has no correlation with LDL.Conclusions:1.The level of Plasma Lp-PLA2 in patients with atherosclerotic vascular vertigo increased.Lp-PLA2 involved in the development of atherosclerosis,suggesting that vascular vertigo may be related to the intracranial arteriostenosis caused by atherosclerosis.2.The level of CRP in patients with atherosclerotic vascular vert igo increased,suggesting that inflammatory response may be involved in the occurrence of atherosclerotic vascular vertigo.3.The level of CRP in patients with atherosclerotic vascular vert igo increased s CD40 L mainly from activated platelets,suggesting that atherosclerotic vascular vertigo may be related to the platelet activati on.4.Hypertension and diabetes are risk factors for atherosclerotic v ascular vertigo.5.The inflammatory response to atherosclerosis may be related t o abnormal lipid metabolism.
