Gastroesophageal reflux disease(GERD)occurs when reflux of gastric contents causes troublesome symptoms or complications,hydrochloric acid in gastric juice,pepsin and bile(such as the combination of bile salt and trypsin)reflux is the direct damage factors.At present,GERD affects about 35% of the world's population health,and its incidence is still growing year by year.According to whether gastroscopy causes esophageal mucosal erosion or ulcer,it can be roughly divided into reflux esophagitis(RE)and nonerosive reflux disease(NERD).Helicobacter pylori is a very common type of pathogen,and more than half of the people in the world have infected the bacterium.Helicobacter pylori(H.PYLORI)infection plays a major role in the pathogenesis of peptic ulcer disease,chronic gastritis,gastric mucosa-associated lymphoid tissue lymphoma(MALToma)and the development of gastric cancer.However,its role in reflux-induced esophageal injury(reflux esophagitis(RE))and gastroesophageal reflux disease without esophagitis(GERD)is not fully understood.And the effect of Helicobacter pylori infection on the severity of reflux esophagitis is rarely mentioned in the literature at home and abroad.The main purpose of this research is through retrospective analysis in First Affiliated Hospital of Hebei North University for upper gastrointestinal endoscopy for the diagnosis of reflux esophagitis and Helicobacter pylori infection rate in patients with chronic gastritis,chronic gastritis and reflux esophagitis in patients with Helicobacter pylori(H.pylori)infection rate,and analysis of the relationship between Helicobacter pylori and reflux esophagitis severity,to investigate the correlation between Helicobacter pylori infection and reflux esophagitis.The experiment selected a total of 210 people come to First Affiliated Hospital of Hebei North University from April 2017 to January 2018 as the object of study.110 people were diagnosed with RE,and we choose 100 people were diagnosed with Chronic gastritis in the same period as a control group.All people were diagnosed by undergoing esophagogastroduodenoscopy.Severity of RE was assessed by Los Angeles classification:48 patients were classified as Grade A:Mucosal breaks confined to the mucosal fold,each no longer than 5 mm.27 patients were classified as Grade B:At least one mucosal break longer than 5 mm confined to the mucosal fold but not 12 continous between two mucosal folds.24 patients were classified as Grade C:Mucosal breaks that are continous between the tops of two or more mucosal folds but involve less than 75% of the circumference.11 patients were classified as Grade D :Mucosal breaks involves at least 75% of the esophageal circumference.Diagnosis of H.pylori infection was made by 14 carbon urea breath test and rapid urease test.Prevalence of H.pylori infection was lower in cases with reflux esophagitis than in ontrols(34.55% vs 65.0%,?2=19.440,P = 0<0.01)and negative associations with H.pylori infection across the grade of esophagitis(41.33% in Los Angeles classification LA-A or LA-B and 20.00% in LA-C or LA-D,?2=4.803,P<0.01).H.pylori infection was inversely associated with the risk and severity of reflux esophagitis,suggesting the organism may have a protective role against RE.digestive disease.
