Objective:To investigate the related factors which can indicate the decrease of bladder detrusor contractility in patients with benign prostatic hyperplasia,so as to improve the ability to identify the decrease of detrusor contractility.Methods:The clinical data of patients who underwent urodynamic examination from January 2013 to December 2016 in the Department of Urology,the First Affiliated Hospital of Jilin University,were reviewed.Patients who were diagnosed prostate cance by postoperative pathology or puncture biopsy were excluded.Only344 patients with benign prostatic hyperplasia were included.The bladder contraction index(BCI)was calculated according to the results of urodynamic test,and devided groups according to the BCI.Put BCI<100 in decreased group with 181 cases,and put BCI?100 in non decreased group with 163 cases.Excel was used to record the data of patients in the two groups,inculding age,size of prostate,urinary retention status,hydronephrosis status,neurological diseases status,whether complicated with hypertension,whether have diabetes and so on.SPSS23.0software is used for analysis.The measurement data were compared by rank sum test and compared counting data by Pearson ?2 test.Multivariate Logistic regression analysis was carried out among those significant risk factors in univariateanaylsis.P<0.05 is considered significant statistically.Results:Age(Decreased group 73.1±8.7years old,Non decreased group 70.7±7.7years old P=0.003);The size of the prostate(Decreased group 60±33.9ml,Non decreased group68.5±29.7ml,P=0.001);Complicated with hydronephrosis(Decreased group 17.7%,Non decreased group 10.4%,P=0.032);Complicated with diabetes(Decreased group 18.8%,Non decreased group 11.0%,P=0.045)?In the multivariate Logistic regression analysis,Age(OR=1.048,95%CI:1.019-1.077,P=0.001),The size of the prostate(OR=0.987,95%CI:0.979-0.994,P=0.000,B=-0.014)complicated with hydronephrosis(OR=2.367,95%CI:1.217-4.602,P=0.011),Complicated with diabetes(OR=2.152,95%CI:1.123-4.123,P=0.021).They are independent factors associated with the decrease of the bladder detrusor contractility,which can be used to predict whether the detrusor contractility is weakened.Conclusion:1.Age,a history of diabetes and a history of hydronephrosis are effective factors to evaluate the ability of the Bladder detrusor contractility in the patients with benign prostatic hyperplasia.Without considering other factors,a patient with advanced age,a history of diabetes or a history of hydronephrosis is more likely to has reduced bladder detrusor contractility.2.Patients with Benign prostatic hyperplasia have the possibility of bladder detrusor contractility decrease,even though their prostate isrelatively small.And they are more likely to hve decreased bladder detrusor contractility than those who have relatively big prostate with similar clinical symptoms.
