Objective:This study tends to assess the efficiency of microsurgical fenestration and endoscopic fenestration for temporal arachnoid cyst,supplying the factual evidence for the first choice of the management of TAC.Methods:Review and analyse the clinical information of 47 TAC patients who have suffered from microsurgical fenestration or endoscopic fenestration,in the first and second ward of neurosurgery of Shengjing hospital which is affiliated to China Medical University from August 2012 to July 2017.Based on the surgical treatment,the patients can be divided into two parts:Endoscopic Group and Craniotomy Group.Compare the gender,the age,the size of cysts and the preoperative function to ensure the comparability of the two groups.Statistical analysis was enforced on the symptom relief rate,the reduction of cyst and the occurrence of postoperative complications in 1 weeks so as to compare the short-term therapeutic effects between the groups.Results:The symptom remission rates of the postsurgery of the craniotomic cyst resection surgery and endoscopic arachnoid cyst fenestration were 96% and 92.3%,P>0.05,no statistical significance.The rates of the recent imaging visible cysts reduction were 13.8% and 0%,P>0.05,thus having no statistical significance.The rates of intracranial infection in the recent postoperative complications were 34.4% and 26.7%,P>0.05,having no statistical significance.The rates of subdural effusion were 3.1% and0%,P>0.05,no statistical significance.The long-term symptom relief rates were 96%and 100%,respectively.There was no statistical significance because P>0.05.After operation,the effective rates of the reduction of the cysts were 46.7% and 42.3%,P>0.05,thus there is no statistical significance.No long-term complication was occured after the operations.The long-term quality scores of life after the operations were improved in different degrees comaring with those of preoperations,and there was no statistical difference between the two types according to P>0.05.Conclusion:Craniotomic cystectomy and endoscopic cystostomy both have good effects in the treatment of temporal arachnoid cysts.They have similar efficiencies in the short and long term.
