Therapeutic Effect Of Craniotomy Cyst Excision And Cyst-Peritoneal Shunt On Arachnoid Cyst

Objective:To compare the effect of cystectomy and cyst-peritoneal shunt in the treatment of arachnoid cyst,and to analyze the effect of the two kinds of surgical procedures.Methods:All 55 patients(36 males and 19 females)were diagnosed as arachnoid cyst in Shengjing Hospital from January 2012 to August 2017.Among them,37 cases(craniotomy group)were given cyst excision and 18 cases(shunt group)were underwent arachnoid cyst-peritoneal shunt.The clinical data of the two groups were compared about both the recent postoperative recovery and the incidence of postoperative complications after 6 months follow-up.Results :There were 37 people in craniotomy group,including 23 males and 14 females,aged 7(5~9)years.16 cases located in lateral fissure,21 cases located in non lateral fissure.There were 18 cases in shunt group,including 13 males and 5 females,6years(2.85~8.75),9 cases located in lateral fissure,9 cases located in non lateral fissure.There was no statistically significant difference in age,sex and cyst location between the two groups(P>0.05).In the craniotomy group,4 patients had no obvious improvement,and all the patients in the shunt group had improvement.There was no statistical difference between the two groups after operation(P>0.05).In the craniotomy group,3 patients had postoperative complications after operation,and all the patients were improved after treatment.2 patients in the shunt group had postoperative complications after the operation.There was no significant difference in postoperative complications between the two groups(P>0.05).After 6 months of follow-up,1 patients in the craniotomy group had a recurrence of symptoms,and improved again after operation.2 patients in the shunt group had a recurrence of symptoms.There was no significant difference in the follow-up results between the two groups(P>0.05).No long-term complications were found in the craniotomy group,and 4 patients in the shunt group had postoperative long-term complications.There was a statistical difference between the two groups(P<0.05).Conclusion : Both craniotomy cyst excision and cyst-peritoneal shunt are two effective surgical procedures for the improvement of intracranial arachnoid cysts.Theoccurrence of recent complications was not statistically significant between two groups.The long-term efficacy of patients in craniotomy cyst excision group is better than that in cysts-peritoneal shunt group.
