Objective: The purpose of this study is to discuss the relationship between the level of non-high-density lipoprotein cholesterol(non-HDL-C)and coronary heart disease(CHD)in young patients. Methods: A total of 196 young patients ranged from 14-44 years old who underwent coronary angiography(CAG)at the Department of Cardiology,Shengjing Hospital of China Medical University from June 2015 to July 2017 were selected as the subjects. For those,132 with CHD(case group)and 64 without CHD(control group),and we collected the general information and the related results of laboratory examination of them.According to Gensini score(GS),we divided the case group into two groups:mild and moderate coronary artery disease(GS 1-59,n=78)and severe coronary artery disease(GS?60,n=54);Pearson correlation analysis was used to analyze the correlation between the level of non-HDL-C and GS.Logistic regression was used to analyze the risk factors of severe coronary lesions. The ROC curve was used to evaluate the optimal cut-off point of non-HDL-C level in predicting severe coronary artery disease. Results:The level of non-HDL-C in patients with CHD were significantly higher than those in control group(P=0.002).The level of non-HDL-C in patients with severe coronary artery disease group were significantly higher than those in mild and moderate coronary artery disease group(P=0.000).The level of non-HDL-C was significantly related to Gensini scores in young CHD patients(r=0.390,P=0.000).Non-HDL-C can be the independent risk factor for evaluating the severity of coronary artery disease(OR=1.060,P=0.048,95%CI:1.001-1.123).The result of ROC curve analysis suggests that the best cut-point of non-HDL-C level in predicting severe coronary artery disease is 138.16mg/dL(3.58mmoL/L),specificity of 0.796,and sensitivity of 0.641.Conclusion:Non-HDL-C is related to the occurrence of coronary heart disease and the severity of coronary artery disease in young patients, elevated non-HDL-C level is the independent risk factors for severe coronary artery disease,When the level of non-HDL-C > 138.16mg/dL(3.58mmoL/L),severe coronary artery disease occurs.
