Objective:The Crouse plaque score was used to quantitatively evaluate the carotid atherosclerosis(CAS),to discusse the correlation between Crouse plaque score and hypersensitive c-reactive protein and cerebral infarction of elderly diabetic cerebral infarction patients,to provide scientific theoretical basis for the clinical early effective prevention and treatment of elderly diabetic cerebral infarction patients.Methods:The study involved 100 patients with diabetic cerebral infarction from October 2015 to February 2017.All patients diagnosed after admission to hospital were divided into three groups:massive infarct(10),small infarct(28)and lacunar cerebral infarction(62)according to CT/MRI examination results.General data collection after admission,peripheral venous blood was collected to detect the hypersensitive c-reactive protein the next day,during the same period,100 diabetic patients without cerebral infarction were collected at the medical examination center.Using Color Doppler ultrasound,we measured and recorded the thickness of each independent bilateral carotid artery plaque and added all measured independent plaque thickness together,then served as Crouse score.The hypersensitive c-reactive protein was measured by latex immunoturbidimetry,and to record the data.All the data was analyzed with SPSS23.0 statistical software.Statistical significance was set at P<0.05.Result:1.The Crouse score of diabetes mellitus with cerebral infarction was significantly higher than that in the control group(P=0.00<0.05),and the Crouse score between the two groups was significantly different.2.The hs-CRP of diabetes mellitus with cerebral infarction was significantly higher than that in the control group(P=0.00<0.05),and the hs-CRP of the two groups was significantly different.3.There were significant differences in Crouse scores between the three groups of patients with massive infarct,small infarct and lacunar cerebral infarction(P=0.007<0.05).4.There were differences in hs-CRP between the three groups of patients with massive infarct,small infarct and lacunar cerebral infarction,but the differences were not significant(0.05<P=0.053<0.1).5.The Crouse score of diabetes mellitus combined with cerebral infarction group was positively correlated with hypersensitive c-reactive protein(r=0.505,P=0.000<0.01).6.There was no significant correlation between Crouse score and hypersensitive c-reactive protein in the control group(r=0.159,P=0.114>0.01).7.The type of cerebral infarction was used as the dependent variable,the Crouse score,hs-CRP as predictive variable,and the area size of cerebral infarction was y,Crouse score was x1,hypersensitive c-reactive protein was x2,and the regression model was y=1.750-0.060x1-0.002x2(P=0.010<0.05).Conclusion:The application of carotid ultrasound Crouse score in quantitative evaluation of carotid atherosclerotic plaques has high reliability.Carotid ultrasound Crouse score and hs-CRP were closely related to the occurrence of cerebral infarction with diabetes mellitus.There was a close relationship between the carotid artery Crouse score and hs-CRP in diabetic patients with cerebral infarction.Crouse plaque score combined with hs-CRP can partly predict the type of cerebral infarction area.
