Objective:To study the expression of cytokeratin 19 in endometrial cancer and its lymph nodes.The relationship between CK19 and surgical staging?depth of myometrial invasion?ascitescytology?infiltration of lymphatic space?tumor differentiation?histological type?tumorvisual classification?postoperativelytreated?hypertension?diabetes?BMI?menopause and so on were analyzed.To investigate the diagnostic value of CK19 on lymph node micrometastasis in endometrial carcinoma.It provides a theoretical basis for clinical diagnosis and individualized follow-up or treatment of patients with endometrial cancer.Methods:1.We retrospectively studied 357 lymph nodes from 42 patients with endometrial cancer undergoing surgical treatment at the First Affiliated Hospital of Shanxi Medical University from January 2015 to December 2016.An average of 8.5 lymph nodes were detected in each patient.22 of them were endometrial carcinoma stage ?,with a total of187 lymph nodes,12 cases of endometrial cancer stage ?,with a total of 102 lymph nodes,and 8 cases of endometrial cancer stage ?C1 with a total of 68 lymph nodes.(Among them,17 lymph nodes are metastatic lymph nodes).In 42 patients,6 cases of adenosquamous carcinoma,2 cases of serous adenocarcinoma,and 1 case of mucinous adenocarcinoma were removed.The remaining 33 cases were all endometrioid adenocarcinoma.The patient's age ranged from 37 to 62 years and the average age was52.3 years.2.?mmunohistochemistry was used to detect CK19 in tumor tissues and corresponding lymph nodes of patients with endometrial cancer.3.Using SPSS 13.0 software,statistical analysis was performed usingc~2 test and Fisher exact probability method,P<0.05 was statistically significant.4.To analyze the expression of CK19 in endometrial carcinoma and its lymph nodes.Results:1.In 42 cases of endometrial cancer,CK19 was positive.2.In 357 lymph nodes,the detection rate of traditional hematoxylin-eosin staining was 4.8%(17/357),and the detection rate of CK19 immunohistochemistry was 16.2%(58/357).The difference between the two examination methods was statistically significant(c_c~2=85.670,P<0.001).3.The expression rates of CK19 in stage ? and stage ? endometrial cancer patients were 18.2%and 58.3%,respectively.The difference was statistically significant(P=0.026<0.05).With the increase of myometrial invasion,CK19 was The positive rate in lymph node tissue increased from 13.6%to 66.7%,the difference was statistically significant(P=0.003<0.05);the expression rate of CK19 was 63.6%in the lymph nodes of patients positive for ascites cytology and negative in cytology of ascites.The lymph node metastasis rate in patients was 17.4%,and the difference was statistically significant(P=0.016<0.05).The positive rate of CK19 in lymph nodes with lymphocytic infiltration was 88.9%,and it was found in the lymph nodes of patients without lymphatic interstitial infiltrates.The positive rate was 12%,and the difference was statistically significant(P<0.001).4.The expression of CK19 in endometrial carcinoma stage ? and ? lymph node tissues was not significantly different from histological type,tumor visual classification,postoperatively treated,hypertension,diabetes,BMI or menopause(P>0.05).5.There was no significant difference in the expression of CK19 in lymph node tissues of endometrioid adenocarcinoma with different degree of differentiation(P=0.118>0.05).Conclusion:1.The sensitivity and specificity of CK19 for endometrial cancer are both high.CK19 can find the micrometastasis of endometrial cancer as early as possible.2.The expression of CK19 in stage ? and ? lymph node tissues of endometrial carcinoma was related to the surgical staging,depth of myometrial invasion,cytology of ascites,and infiltration of lymphatic space but not related to the degree of tumor differentiation,histological type,tumor visual classification,postoperativelytreated,hypertension,diabetes,BMI or menopause.
