Objective:Urea,serum creatinine(SCr)and endogenous creatinine clearance rate(CCr)are commonly used as indicators for evaluating renal function in clinic but without enough sensitivity and specificity.In this paper,through a retrospective study to analyze the changes and related factors of serum cystatin C(Cys-C)levels in children with primary nephrotic syndrome(PNS)and to analyze its clinical significance,so as to heip the early diagnosis of renal injury in children with PNS.Methods:Clinical date of 50 patients with PNS diagnosed in Pediatric First Hospital of Shanxi Medical University from March 2014 to November 2017 were collected and as nephropathy group,50 healthy children who underwent physical examination were enrolled as the control group.The date of the control group collected includes the serum levels of Cys-C,urea,SCr.The date of the control group collected includes the serum levels of Cys-C,Urea,SCr,albumin(ALB),total cholesterol(TC),triglyceride(TG),uric acid(UA),24 h urine proteinquantitation(24hUpr),fibrinogen(Fib),D-dimer(D-d),thrombin time(TT),prothrombin time(PT),activated partial thromboplastin time(APTT),fibrin degradation product(FDP),antithrombin ? activity(AT-?A),platelet count(PTC),WBC,mean platelet volume(MPV).The date collected in the study all use the unified test results of the Laboratory of the First Hospital of Shanxi Medical University.SPSS22.0 software was used to analyze the difference of Cys-C,Urea and SCr levels between the nephrotic group and the control group.The correlation between the Cys-C level and the aboveindicators was analyzed,and the data significance was analyzed combined with clinical data.Results:1.The Cys-C(mg/L)in the nephropathy group and the control group were 1.05 +0.48 and 0.61 + 0.11 respectively,the difference between the two groups was statistically significant(P<0.01),and there was no significant difference in Urea and SCr between the two groups(P>0.05).2.PNS in children with Cys-C,Urea,SCr were positively correlated.(r was0.559,0.613 respectively,P<0.001).3.PNS in children with Cys-C,serum UA,24 hUpr were positively correlated.(r was0.429,0.457 respectively,P<0.01).Conclusion:1.For the evaluation of renal function in children with PNS,Cys-C was significantly superior to Urea,SCr,or may be used as an ideal biomarker for the early diagnosis of renal injury in PNS;2.The different levels of Cys-C may be used as indicators to assess the extent of glomerular filtration membrane damage in PNS;3.Cys-C combined with serum UA levels may provide a more reliable indicator for clinical evaluation of glomerular filtration function in PNS.
