Objective:The object of this research is to use proteomic technologies to separate and identify the protein mapping with significance deviation of patients who suffered from diabetes mellitus type 2 with different syndromes in order to build a protein-molecule diagnostic model of the patterns of syndrome in TCM of diabetes mellitus type 2,especially the micro changes of asthenia syndromes of lien and kidney.This research can improve the study of incorporating the disease and pattern of diabetes mellitus type 2,lay a foundation of the standardization and objectification of clinical differentiation of symptoms and signs for classification of syndrome,and promote the diagnosis and treatment of diabetes mellitus type 2 with the method of TCM.Method:According to the diagnostic criteria of this study,70 patients suffered diabetes mellitus type 2 were selected and grouped by TCM syndrome differentiation in accordance with the relevant TCM syndrome differentiation standard.There were 40 cases in spleen deficiency group,12 in kidney deficiency group,18 in the other group,and 45 normal subjects.The method of grouping is to combine the disease with pattern: 1)diabetes-mellitus-type-2 group vs.normal control group: comparatively analyze the protein mappings between 70 diabetes-mellitus-type-2 patients and normal persons,and establish the diagnostic model of salivary proteins in type 2 diabetes mellitus "disease";2)the group of patients who suffered this disease with different syndromes: divide type 2 diabetes mellitus on the basis of syndrome differentiation of Chinese medicine standards into spleen deficiency group,kidney deficiency syndrome group and other groups,and analyzed them compared with the normal control group,hence establish salivary protein diagnosis model of type 2 diabetes "syndrome";3)Comparison of different type 2 diabetes TCM syndrome group:compare the different express screening protein peaks between spleen deficiency group,kidney deficiency syndrome group and other groups each other.Result:1)the type 2 diabetes group and normal control group: Using statistical significance protein peak 2082.92m/z?2499.24m/z and 2426.77m/z to build and verify the diagnostic model,it is found that the sensitivity of type 2 diabetes group and normal control group is 94.3%(66/70),with the specificity being 93.3%(42/45),after double-blind randomised trials,the sensitivity of type 2 diabetes group and normal control group is 88.6%(62/70),with the specificity being 77.8%(35/45);2)The spleen deficiency group of type 2 diabetes mellitus and normal control group: Using statistical significance protein peak 2082.92m/z and 2843.79m/z to build and verify the diagnostic model,it is found that the sensitivity of this diagnostic model is 97.5%(39/40),with the specificity being 86.7%(39/45),and after double-blind randomised trials,the sensitivity is 85%(34/40),with the specificity being 82.2%(37/45);3)The type 2 diabetes spleen kidney deficiency syndrome group and the normal control group: Using statistical significance protein peak 5744.34m/z and 2410.88m/z to establish and verify the diagnosis model,it is found that the sensitivity of type 2 diabetes mellitus group of TCM syndrome type of the subclass is 91.7%(11/12),with the specificity being 95.6%(43/45),after double-blind randomised trials,the sensitivity is 83.3%(10/12),with the specificity being 75.6%(34/45);4)other group of the type 2 diabetes and the normal control group: Using statistical significance protein peak 2082.92m/z and 3036.77m/z to establish and verify the diagnosis model,it is found that the sensitivity of type 2 diabetes mellitus group of TCM syndrome type of the subclass is 94.4%(17/18),with the specificity being 95.6%(43/45),after double-blind randomised trials,the sensitivity is88.9%(16/18),with the specificity being 93.3%(42/45).5)Compared with each other between the different TCM syndrome group: compared with the type 2 diabetes kidney and spleen deficiency syndrome,the different protein peaks are 304,which was a significant difference significance(P <0.05)are 9;there are 304 differentially expressed protein peaks between spleen deficiency syndrome group and other groups,and 14 was statistically significant(P <0.05);kidney deficiency syndrome group and other groups,was found 301 differentially expressed protein peaks,and 2 was significantly differentially expressed(P <0.05).Conclusion:In this study,the liquid chip combined with MALDI technique for the detection of the salivary protein fingerprint atlas of the spleen and kidney deficiency syndrome as well as other syndrome type 2 diabetes patients from the normal people.Then choose the most meaningful specific biological markers from salivary proteins screened and establish relevant combination of disease and syndrome diagnosis models,which can distinguish correctly in patients with type 2 diabetes and different TCM syndrome type of patients from the normal people,this salivary protein fingerprint atlas detection technique has very important clinical significance and opens up a new way for TCM clinical diagnosis of diseases and dialectical diagnosis,as well as provides the basis of the TCM microcosmic dialectic researches.
