Objective: Studies have reported that the occurrence and development of colorectal cancer may be related to oxidative stress, but most previous studies detecting one or several kinds of oxidant or antioxidant markers failed to consider the total oxidant/antioxidant status of the body, which couldn’t fully reflect relations between oxidative stress and pathogenesis of colorectal cancer. Therefore,in order to investigate the relationship between the overall level of oxidative stress and the occurrence and development of colorectal cancer,we detected and compared oxidative stress parameters in serum of colorectal cancer patients and healthy subjects. At the same time to analyze its clinical value in the diagnosis of colorectal cancer. Methods: 132 patients with colorectal cancer and 64 healthy subjects were collected fasting blood, and measured oxidative stress parameters that included total oxidant status(TOS),total antioxidant status(TAS)and calculation of oxidative stress index(OSI).In the case of no significant difference in basic characteristics of colorectal cancer and healthy subjects,we compared the difference of oxidative stress parameters. This study also compared the oxidative stress parameters in colorectal cancer with different clinical classification, different primary sites, the history of smoking and drinking, tumor status, preoperative and postoperative, different metastasis.Statistical processing: Basic characteristic between colorectal cancer patients and healthy subjects in age was compared by t test.Gender and the history of smoking and drinking were compared by chi-square test.Whether oxidative stress parameters(TOS,TAS and OSI)accorded with normal distribution by Kolmogorov-Smirnov test.If the results of the samples were normal distribution, the results with the mean ±standard deviation, comparison between the two groups using t test, comparison between multiple sets of the analysis of variance(F test). If sample data were skewed distribution, the results with median(P25 ~ P75),using the Kruskal-Wallis test(expressed in chi-square statistics). The diagnostic performance of oxidative stress parameters in colorectal cancer was analyzed by the receiver operating characteristic curve(ROC).Using IBM SPSS 22.0 statistical software for statistical analysis,P < 0.05 as statistically significant. Results: comparison of basic characteristics of 132 cases of colorectal cancer patients and 64 cases of healthy subjects had no statistical difference, the TOS,TAS and OSI in these two groups were statistically significant(P = 0.000),and TOS,OSI levels in colorectal cancer increased significantly,TAS levels decreased significantly. The study found that oxidative stress parameters were significant difference in different patients with smoking and drinking(P < 0.05).Comparison of oxidative stress parameters in different primary sites(colon / rectum),tumor status and clinical staging showed no statistical significance(P > 0.05).Preoperative and postoperative oxidative stress parameters in the same patients with colorectal cancer were no significant difference(P > 0.05).this study also found that oxidative stress parameters no statistically significant difference between no metastatic and metastatic patients(P > 0.05),while no metastatic patients compared with patients for two parts of metastases, and the results were statistically significant difference(P < 0.05).ROC curve analysis showed that the AUC of TOS,TAS and OSI was 0.783,0.853,0.844,and their Youden index was 0.447,0.597,0.562,respectively.Conclusion: oxidative stress parameters in serum are related with the occurrence of colorectal cancer, and may be associated with disease severity in patients with colorectal cancer.Smoking and drinking affect the oxidative stress state in the body.Oxidative stress parameters in the diagnosis of colorectal cancer have certain reference value, but the specificity is not high.
