A Clinical Research Of The Plasma Oxidative Stress Level In Preschool Children With OSAHS

Objective This subject based on the local and international studies on close relationship between obstructive sleep apnea/hypopnea syndrome(OSAHS) and the oxidative stress, and selected 3-7 years old preschool OSAHS patients as the main research object, by detecting the expression of plasma total antioxidant capacity(T-AOC)、heat shock protein 70(HSP70)and superoxide dismutase(SOD),to explore the oxidative stress state in preschool children with OSAHS, and analyze the correlation between the different severity of OSAHS and oxidative stress state and its clinical significance, to provide a certain basis for the diagnosis, treatment and prognosis evaluation of children with OSAHS in the future.Methods At the department of Otolaryngology Head and Neck in children’s hospital of Hunan province from 2014.1 to 2014.12, there were 105 preschool 3-7 years old OSAHS patients as the experimental group, 79 cases were male, 26 were female,the average age was(55.04 + 11.60)months. They were diagnosed by continuously more than 7 hours night sleep figure Polysomnography(PSG) 、Nasopharyngeal lateral slice, electronic nose pharynx endoscopy during their hospitalization for bilateral amygdala stripping operation under general anesthesia and(or) the nasal endoscopic adenoidectomy. According to the PSG experimental group can be divided into mild group(n=30),moderate group(n=40) and severe group(n=35).While preschoolers 25 cases who were diagnosed congenital aural fistula and congenital vice auricle under treatment in hospital as the control group(no sleep snoring and no history of upper respiratory tract disease) from the same department at the same time. The male were 20, female were 5,the average age was(56.20±10.00) months. All the children’s physical indexes such as age,height, weight, body mass index showed a situation comparable between the two groups subjects.By detecting the expression of the serum specimens T-AOC,HSP70 and SOD,to analyze if there is a correlation between the three above indicators and the severity of disease in children with OSAHS,and analyze the correlation between T-AOC,HSP70 and SOD.Results 1 、 Compared with control group, preschool children OSAHS experimental group plasma T- AOC, SOD level respectively were(7.20 + 0.92)(U/ml),(79.88-12.91)(U/ml),while the control group were(9.18 +0.70)(U/ml),(102.06-3.58)(U/ml),the level of the experimental group significantly decreased than the control group(P < 0.05).Children OSAHS group HSP70 levels significantly increased than the control group, respectively were(183.38 + 18.86)(ng/ml),(168.64-0.96)(ng/ml), after the comparative analysis P < 0.05. 2、Children OSAHS group was divided into mild OSAHS group, moderate OSAHS group and severe OSAHS group,and compare to plasma T- AOC, HSP70 and SOD level between three groups.T- AOC level between the three groups respectively were(8.36 + 0.25)(U/ml),(7.43-0.36)(U/ml),(6.15-0.35)(U/ml), P < 0.05, with statistical significance.HSP70 level between the three groups respectively were(173.95 + 4.19)(ng/ml),(184.85-3.43)(ng/ml),(193.56-5.49)(ng/ml), P < 0.05,with statistical significance. SOD level between the three groups respectively were(93.76±7.77)(U/ml)、(81.79±4.56)(U/ml)、(65.24±3.90)(U/ml), P < 0.05,with statistical significance. 3、Preschool children OSAHS group PSG detection indexes : mild OSAHS group AHI(7.52 + 1.64)(time/hour), La SO2(89.86 + 2.51) %, moderate OSAHS group AHI(16.55 + 2.89)(time/hour), La SO2(81.62 + 3.14) %, severe OSAHS group AHI(35.14 + 11.23)(time/hour), La SO2(69.28 + 5.27) %, and through the analysis p < 0.05,with statistical significance.4 、 Preschool children’s T-AOC and AHI was obvious negative correlation, and La SO2 was positive correlation.SOD and AHI significantly negative correlation, and La SO2 performed significantly positive correlation.HSP70 was positively associated with AHI, and La SO2 correlation analysis showed the negative correlation. 5、Preschool children OSAHS T-AOC, SOD and HSP70 relevant analysis and comparison between three groups. T-AOC and HSP70 showed the negative correlation. T-AOC and SOD showed the positive correlation. SOD and HSP70 showed the negative correlation.Conclusions Compared with control group, there were obvious differences of correlation index reflects the oxidative stress level T-AOC, HSP70, SOD in preschool children with OSAHS, which embodied in the total antioxidant levels index T- AOC significantly declining, organism in body antioxidant capacity index SOD obviously decreased, the oxidative stress level enhancement representative HSP70 expression significantly. There is closely relationship between T-AOC, HSP70, SOD level in plasma of preschool children OSAHS group and AHI, La SO2 reflecting OSAHS severity. Mutual dependence of Children participants T-AOC, HSP70 and SOD in plasma between each other, can show the progress of the disease in some degree. In a nutshell, oxidative stress status is closely related to the correspondingindex changes significantly in preschool children with OSAHS, show correlation between oxidative damage degree and the severity of the condition of OSAHS children.
