Effects Of Concentrated Growth Factors On The Guided Bone Regeneration With Single Dental Implant In The Esthetic Zone

Purpose:To provide some references for clinicians to choose the barrier membrane during the guided bone regeneration,by evaluating of the effects of CGF membrane on the guided bone regeneration with single dental implant in the maxillary esthetic zone.Materials and Methods:A total of 14 cases of planting single anterior teeth and application of concentrated growth factors for guided bone regeneration with simultaneous implant placement were collected from July 2015 to December 2017 at the Stomatology Hospital of Shandong University.a)The survival rate of implants was calculated.b)The thickness of labial bone plate on the implant shoulder platform,1mm below the shoulder stage,middle of the implant,and the root of the implant was measured.c)The score of pink aesthetic index(PES)and white aesthetic score index(WES)were evaluated.d)The postoperative pain degree,swelling degree and patient satisfaction was compared to those received GBR with collagen membrane.The data were analyzed with SPSS 23 software package.When P<0.05,the difference was considered statistically significant.Results:All planting sites healed well and achieved osseointegration.No complication was occurred within three months of the prosthesis and the survival rate of the implant was 100%.At the time of 6 month after implantation,the thickness of the labial bone plate at the shoulder site,1mm below the shoulder,the middle part of the implant and the apical of the implant were 1.61± 0.46 mm,1.98±0.44 mm,2.31±0.55 mm,2.10±0.89 mm,respectively.。When the prosthesis was first worn,the pink aesthetic score(PES)was 9.43±1.35 and the white aesthetic score(WES)was 7.93 ± 1.05.Three months later,the pink aesthetic score(PES)was 10.71 ± 1.63,which was significantly improved comparing with the initial,and there was significant statistical difference(P<0.05).The VAS scores of patients in CGF group and collagen membrane group were 35.1 ± 22.5 and 47.0 ± 20.3 respectively.After grading the degree of pain according to VAS score,the percentages of no pain,mild pain,moderate pain and severe pain in CGF group were 7.1%,57.2%,28.6%and 7.1%,followed by 0.0%,50.0%,42.9%and 7.1%in the collagen membrane group.The pain in CGF group was slightly lighter than that in collagen membrane group,but there was no significant difference(P>0.05).The duration of postoperative pain in the two groups was 2.1 + 1.5 days,2.8 + 1.0 days,and there was no statistically significant difference(P>0.05).The percentages of non-swelling,mild swelling,moderate swelling and severe swelling in CGF group were 21.4%,57.2%,21.4%and 0.0%respectively,and those in collagen membrane group were 7.1%,35.7%,35.7%and 21.4%.The duration of swelling was 2.4 ± 1.4 days and 4.2 ± 2.2 days respectively.There was statistical difference in the swelling degree of CGF group and collagen membrane group(P<0.05).Both patients in CGF group and collagen membrane group had excellent patient satisfaction(91.9 ± 4.8 and 91.3 ± 7.6 respectively),with no significant difference(P>0.05).Conclusion:In the short term,the use of concentrated growth factors in the guided bone regeneration can obtain excellent implant survival rate,pink and white aesthetic effect.And the long-term effect needs to be further observed.The ideal thickness of the labial bone plate can be obtained after the implant osseointegration.The use of CGF can significantly reduce the degree of postoperative swelling and shorten the postoperative swelling time,but there is no significant effect on the pain.
