1.ObjectiveTo get the information of the basic situation and training needs of those working staff who take on health education and their grasp about basic health knowledge in the community medical institutions in Karamay City,to carry out health education service quality evaluation of basic national public health service projects in the community medical institutions,and to analyze the existing issues of health education services in the community medical institutions,and so as to provide the basis for regulating the development of health education services and formulation of related policies.2.Methods2.1 Human resources for health education development:A combination of questionnaires and qualitative interviews.Questionnaire surveys use on-site surveys and qualitative interviews use panel interviews to conduct face-to-face discussions.2.2 Service quality evaluation on health education:Using the relevant evaluation tables in the "Health Education Service Workflow and Evaluation Guideline" developed by China Health Education Center,adopting the method of rapid scoring,and to evaluate service quality and dissemination effect based on five items of health education service included in the "National Basic Public Health Service Project".2.3.Data analysis.Qualitative information is descriptively analyzed and summarized,and quantitative information by descriptive statistics covers the sociodemographic characteristics,training needs,grasp about basic health knowledge,evaluation on health education quality,etc.Comparison was made among residents from different regions on their satisfaction with health education service through variance analysis or Kruskal-Wallis rank sum test according to the sourcing of basic information.3 Results3.1 Current status of health education human resources.3.1.1 Basic information of Working Staff in the community medical institutions There are 310 workers engaged in health education,accounting for 66.2%of the respondents.Male/female ratio is 1:3.43;age ranges from 40 to 50 years old,totaling 152(accounting for 49.0%);academic qualifications are mainly undergraduates,totaling 164(accounting for 52.9%);education majors are mainly nursing and clinical medicine,clinical medicine 105(33.9%)and nursing 132(42.6%);the professional titles are mostly junior titles,and the ratio of high,middle and junior professional titles is 1:4.6:6 respectively.3.1.2 Training requirements for health education personnel.In the past three years(2014-2016),163 people(52.6%)participated in health education-related training.They want to learn about basic knowledge on health education and the related skills,diseases,and project management.The preferred training methods include on-site teaching,combination of teaching and practice,and on-site internship.3.1.3 Basic health knowledge evaluation results.In the evaluation test on basic health knowledge,380(81.2%)scored over 60 points,23(4.9%)scoring over 80 points.The lowest score is 28 points(in Urqin District),the highest score 84 points(in Kelamayi District),and the average score 66.2 points.3.2 Interview resultsThe five managers and 30 business staff who participated in the discussion all stated that the development of health education has a positive effect on the development of their community and plays a positive role in promoting health awareness among residents,understanding their own health status and changing their unhealthy behaviors.Those interviewed exchange their opinion about the main problems in the development of health care education in the community medical institutions such as those receiving the health education from community health care institutions limited in amount,the heavy workload and high pressure of the working staff,and the welfare of working staff job to be improved,and give their suggestion on these existing issues.3.3 Evaluation on Health education service quality3.3.1 Health Education leaflets or folders quality assessment.26 health education leaflets,13 folders,totally 39 Printed materials are collected.The full score of the leaflet or folder quality evaluation is 60 Points.The average score of the 39 leaflets/folders quality assessment is 49.15±2.07 Points,and the average score rate is 81.9%.Among them,7.7%(3 Pieces)of the materials were recognized as "unqualified",79.5%(31 Pieces)of the materials as "qualified",and 12.8%(5 Pieces)of the materials as "excellent".The 185 residents scored an average of 20.29 ± 2.71 for each item of the health education plane communication data,with an average score of 81.2%.Each project score is "qualified."3.3.2 Health Education Brochure Quality Evaluation.Totally 26 Pieces of health education booklets are collected and the full score for the quality assessment is 75.The average score of the quality assessment of the 26 health education booklets are 58.31±3.46 Points,and the average score rate was 77.7%.Among them,11.5%(3 Pieces)of the booklets are titled as"unqualified",73.1%(19 Pieces)as "qualified",and 15.4%(4 Pieces)as "excellent".3.3.3 Quality evaluation of health education audiovisual materials.26 health education audiovisual materials are collected and the full score of the quality assessment is 55 Points.According to the result,the average score is 47.46±2.04,and the average score rate is 86.3%.Among the samPles,61.5%(16 Pieces)of the materials are assessed as "qualified",38.5%(10 Pieces)of the materials as "excellent" and no identified as "unqualified".3.3.4 Health Education Bulletin Quality Evaluation.Twenty-six health education boards are extracted and the full score of the assessment is 75 Points.The average score of the quality assessment for the 26 bulletin boards is 63.58±1.84 Points,and the average score rate is 84.8%.Among them,80.8%(21 Pieces)of the samPles are identified as "qualified",19.2%(5 Pieces)as"excellent" and no recognized as "unqualified”.The 185 residents scored an average of 15.16±1.91 for each item in the health education promotion column,with a score rate of 75.8%.Among them,the overall evaluation and the illustration were self-explanatory,and the interest was strongly rated as "qualified".The content was easy to understand.Easy to understand and content practicability evaluation as "unqualified."3.3.5 Quality assessment of Public health consultation activities.26 Public health consultation activities are collected and the full score for the quality assessment is 14 Points.The average score for the quality assessment of the 26 Public health consultation activities is 13.38±1.50 Points,and the average score rate is 95.6%.Among them,3.8%(1 Pieces)of the activities are recognized as "unqualified",38.5%(10 Pieces)of the activities as "qualified," and 57.7%(15 Pieces)of the activities as "excellent."The 261 residents scored an average of 11.82±1.64 for each project of public consultation activities,with an average score of 78.8%,and the evaluation scores for each project were all "qualified."3.3.6 Quality evaluation of lectures on health education.26 lectures are collected.The full score of the lectures in quality evaluation is 75 Points.The average score of the 26 lectures in quality assessment is 60.69±5.00 Points,and the average score rate is 80.9%.Among them,7.7%(2 acts)are identified as "unqualified",73.1%(19 acts)as "qualified",and 19.2%(5 acts)as"excellent".The average evaluation of health knowledge lectures by 248 residents was 16.42 ±1.72,with an average score of 82.1%.Each project was rated as "qualifie.3.3.7 Individualized health quality assessment.157 Pieces of individualized health education materials are collected.The full score for the assessment is 25 Points.The average score of the 157 Pieces of individualized health education materials for quality evaluations is 13.88±1.25 Points,and the average score rate is 55.5%,of which,89.2%(140 coPies)of the materials assessed as "unqualified",10.8%(17 coPies)as "qualified" and no as "excellent".4 Conclusion4.1 Human resources for health education:In the community health care institutions of Karamay City,the working staff are with heavy workload and there are not enough full-time workers.The structure of qualification titles of the working staff is irrational and high-level talents are deficient.the age structure of the working staff is irrational and there is a gap of the workers' age.the composition of professional majors is unreasonable and professionals majoring in public health are deficient.the working staff need to improve their profession in the health education to be excellent and their technical skills need to be enhanced greatly.4.2 The quality of national basic public health services health education services:the qualification rate of the health education service quality is relatively high,but the quality of individualized health education services is relatively low.5 SuggestionStrengthen government functions and attach importance to the development of health education;Optimize staffing and stabilize grassroots talents;Strengthen training and improve the service level of working staff in community medical institutions.
