Objective: To investigate the optimal cut-point value of HbA1 c in the diagnosis of diabetes mellitus and pre-diabetes of Xinjiang uygur nationality,and to compare the differences between different places.Methods: According to the epidemiological survey data of 1,766 uygur residents in Erdaqiao community(city)of Urumqi and 1,296 permanent residents in Tokxun county(rural area)of Turfan city from August to December 2015,the ROC curve was drawn to determine the optimal tangential value of HbA1 c.Results: With OGTT as the gold standard,a total of 234 people were newly diagnosed with diabetes in both places,accounting for 8.09% of the total number of valid data.The optimal cut-off point of HbA1 c for the diagnosis of diabetes mellitus(DM)was6.05%,with a sensitivity of 91.9%,a specificity of 82.4%,and a Youden index of 0.743.The area under the ROC curve(AUC)was 0.922(95% CI:0.911~0.933),a total of 291 people were newly diagnosed with pre-diabetes in the two places,accounting for 10.07%of the total effective data.The optimal cut-off point of HbA1 c in the diagnosis of pre-diabetes was 5.75%,with a sensitivity of 86.3%,a specificity of 70.2%,a Youden index of 0.565,and an AUC of 0.805(95% CI: 0.784~0.927).The optimal tangential value of HbA1 c for the diagnosis of urban and rural diabetes is 6.05% and 5.95%,respectively.The optimal tangential value for the diagnosis of urban diabetes is equal to the optimal tangential value for the diagnosis of diabetes in both places,higher than that in rural areas.The optimal tangential value of HbA1 c for the diagnosis of pre-diabetes in urban and rural areas was 5.45% and 5.75%,respectively.The optimal tangential value for the diagnosis of impaired glucose regulation in rural areas was equal to the two places and higher than that in urban areas.Conclusion: HbA1 c has high cut-off point of sensitivity and specificity in the diagnosis of diabetes with uygur nationality,Xinjiang,had high clinical reference value.But had a low sensitivity and specificity in the diagnosis of pre-diabetes.Currently,Fasting Plasma Glucose(FPG)and Hous Postprandial Blood Gloucose(2h-PG)still need to be combined for diagnosis uygur nationality.
