Objective: to investigate the the role and clinical significance of Vascular endothelial growth factor 121 and Matrix metalloproteinase-2 of the two genes in lung cancer occurrence and development process with pleural metastasis.Methods: 105 cases of patients with pleural effusion from lung cancer hospital of Xinjiang Medical University from October 2016 to October 2018,from above,patients in medical thoracoscopic parietal pleural biopsy and specimens from pleural effusion samples,pathologically diagnosed 49 cases of malignant pleural lesions and 56 cases of benign pleural disease.The expression level of VEGF 121 and MMP-2mRNA in pleural tissue and pleural effusion was detected by real-time fluorescence quantitative PCR(Quantitative Real-time PCR).Results :(1)the relative expression levels of VEGF121 mRNA in benign and malignant pleural tissues were 1.029 + 0.175 and 1.464 + 0.329,respectively,and the difference was statistically significant(t=-8.599,P<0.01).The relative expression levels of VEGF121 mRNA in benign and malignant pleural effusion were 1.036 + 0.187 and 1.450+ 0.330,respectively,and the difference was statistically significant(t=-8.036,P < 0.01).The relative expression levels of MMP-2 mRNA in benign and malignant pleura were1.050+ 0.209 and 1.687+0.604,respectively,and the difference was statistically significant(t =-7.571,P< 0.01).The relative expression levels of MMP-2 mRNA in benign and malignant pleural effusion were 1.011+0.189 and 1.606+0.386,respectively,and the difference was statistically significant(t =-10.219,P <0.01).The relative expression levels of VEGF121 and MMP-2 genes in malignant pleural tissue and pleural effusion were significantly higher than those in the benign control group(P<0.01).(2)there was a positive correlation between the mRNA expression of VEGF121 and MMP-2 in malignant pleural tissue and pleural water(r=0.379,P < 0.05).The sensitivity and specificity of the combined detection(parallel test)were 87% and 96%,and the sensitivity and specificity of the combined detection(tandem test)were 41% and 100% respectively.Conclusion:high expression of VEGF121 mRNA and MMP-2 mRNA in malignant pleural effusion and pleural effusion may be associated with the occurrence and development of pleuralmetastasis of lung cancer.Combined detection of VEGF121 mRNA and MMP-2 mRNA can improve the sensitivity and specificity,and has important clinical value in differentiating benign from malignant pleural effusion.
