| Background:With the continuous aggravation of the aging of society and the sharp rise of the elderly population,the health problems of the elderly deserve our attention.Gastroesophageal reflux disease is a common disease of the digestive system.The number of elderly patients with gastroesophageal reflux disease increases year by year,seriously affecting the quality of life of the elderly.The elderly is different from the general population in terms of physiology,psychology and other aspects,and clinical treatment is more difficult.Objective:The purpose of this study was to observe the clinical efficacy and drug safety of Shugan Hewei combined Western medicine in the treatment of gastroesophageal reflux disease in the elderly,and to provide theoretical support and treatment reference for the clinical treatment of elderly patients,in order to alleviate the suff ering of elderly patients,improve their quality of life,and provide better medical assistance to elderly patientsMethods:70 cases of senile gastroesophageal reflux disease were collected from outpatient department who met the criteria of inclusion and did not meet the exclusion criteria,and were divided into two groups according to the order of entry into the study.In the treatment group,35 cases were treated with the method of Shugan Hewei combined with rabeprazole,with 1 dose of traditional Chinese medicine per day,taken in the morning and evening,and 10mg of western medicine per time and 1 time per day.The control group(n = 35)was treated with rabeprazole,the same as the treatment group.Eight weeks for 1 sessions,the two groups of 1 courses before and after treatment of patients with reflux disease Questionnaire RDQ,gastroesophageal reflux disease health-related quality of life scale(GERD-HRQL)of the total score.The changes of TCM symptom total score,main symptom score and secondary symptom score were observed before and after treatmentResults:1.Reflux disease symptoms change:RDQ integral before and after treatment two group were lower than before treatment(P<0.05),the treatment group after treatment of 28 cases of RDQ score<12 points,while the control group of 15 cases,compared two groups of patients after treatment of RDQ integral,the treatment group than controls and the difference was statistically significant(P<0.05),the treatment group after treatment to the improvement of the reflux symptoms than the control group.2.Improvement of RDQ level:the total effective rate of the treatment group and the control group after treatment was 97.06%and 75.76%,respectively,and the difference was statistically significant(P<0.05).In ter:ms of improvement of RDQ level,the treatment group was significantly better than the control group,and the total effective rate was significantly higher than the control group.3.Improvement of quality of life:the total gerd-hrql score after treatment in both groups was lower than that before treatment(P<0.05),and the total gerd-hrql score in the treatment group was lower than that in the control group(P<0.05).The dif'ference between the treatment group and the control group was statistically significant,indicating that the quality of life of patients in the treatment group was higher than that in the control group.4.Improvement of TCM symptoms:the total score of TCM symptoms before and after treatment in both groups was lower than that before and after treatment(P<0.05),and the total score of TCM symptoms in the treatment group was lower than that in the control group(P<0.05),showing a statistical difference,indicating that the improvement of TCM symptoms in the treatment group was superior to that in the control group.5.The four primary symptom of traditional Chinese medicine to improve the situation:the two groups before and after the primary symptom of TCM treatment from the previous single integral decreased(P<0.05),the treatment group in improving the anti acid,breast bilges full,abdominal distention were significantly better than that in control group(P<0.05),the treatment group and control group for these symptoms have improved,but the treatment group were superior to control group,and both comparisons on belching no statistical difference(P>0.05),shows that the two groups all have positive effect to the improvement of the belching aspects.6.The doctor of traditional Chinese medicine syndrome integral comparison:two groups before and after treatment of TCM syndrome integral and all the previous decreased(P<0.05),after treatment,the treatment group of TCM syndrome integral and lower than that of control group(P<0.05),the treatment group in improving mood not free is aggravating were significantly better than that in control group(P<0.05),while in the improvement of poor appetite,nausea vomiting,bosom is frowsty xi heave a deep sigh,two groups have no significant difference(P>0.05),treatment group and control group have a positive effect on the improvement of the symptoms.7.Evaluation of curative effect of traditional Chinese medicine:the total effective rate of the treatment group was 97.06%,and that of the control group was 66.67%.Compared with the treatment group,the effective rate of the two groups was significantly higher than that of the control group(P<0.05)8.After drug withdrawal for 4 weeks,the recurrence rate of the treatment group was lower than that of the control group(P<0.05),and the efficacy of the treatment group was more lasting with less recurrence.9.During the treatment process,patients in the treatment group and the control group had no obvious toxic and side effects and adverse reactions,so this clinical medication had good safety.Conclusion:The combination of liver and stomach therapy and western medicine in the treatment of gastroesophageal reflux disease in the elderly with disharmony between liver and stomach has significant clinical efficacy,improving reflux symptoms and TCM symptoms,and improving the quality of life of the elderly.This method is safe and effective,no obvious side effects on the elderly,so the clinical treatment of this disease is worth reference. |
