OBJECTIVE:To observe the clinical efficacy of pure Medlite C6 Q-switched Nd:YAG1064nm laser in the treatment of Melasma and auricular acupoint pressing combined with Medlite C6 Q-switched Nd:YAG 1064 nm laser for treating melasma,and compare the therapeutic effects and patient life quality of two different regimens.To improve the quality,we tried to find a comprehensive treatment method with high efficacy and small side effects for the treatment of melasma with integrated traditional Chinese and modern medicine,and explored its possible mechanism of action.METHODS:From May 2018 to February 2019,59 women aged 25 to 50 years old who were diagnosed with melasma at the Laser Beauty Center of the Central Hospital of the Chinese People's Liberation Army were selected as subjects.They were randomly divided into two groups: pure laser group and laser combined with auricular acupuncture group.29 patients were included in the laser alone group,and 30 patients were included in the combined group.The laser group only received Medlite C6 Q switch Nd:YAG 1064 nm laser treatment,1 time per month,for a total process of 4 treatments,therefore a whole observation course are 16 weeks.The combined group according to TCM classification for three types,dialectical pressure points,each will be changed once a week(shorter in summer)and switch between left and right ears,16 weeks for an observation course.Photographs were recorded before and during each treatment,and three dermatologists who did not participate in the treatment compared and recorded the modified Melasma-Area-and-Severity Index(m MASI)scores before and after treatment.Before treatment and at follow-up,patients were asked to record the Melasma Quality of Life Scale(Melas QOL),and the data of m MASI and Melas QOL scores before and after treatment were analyzed by SPSS.All adverse events occurred during the treatment should be recorded.Result:1.All 59 patients were enrolled in the treatment and follow-up as required.2.At 4 weeks after the end of the treatment,the difference of m MASI between the laser group and the combined group was respectively 1.59±0.68 and 3.91±1.16,The m MASI difference between the two groups was statistically significant(p<0.05).3.The Melas QOL scores of the laser group and the combined group were respectively 33.17±4.67 and 24.83±7.02.The Melas QOL difference of scores between the two groups was respectively 6.52 ± 2.41 and 13.40 ± 4.51.The Melas QOL scores after treatment and the difference in Melas QOL scores before and after treatment were statistically significant(p<0.05).4.There were no significant adverse reactions such as blisters or pigmentation were ever observed in both groups during this observation.Conclusion:1.Q-switched Nd:YAG 1064 nm laser and auricular acupressure therapy can effectively treat melasma.2.The clinical efficacy of auricular acupoint pressing combined with Q-switched Nd:YAG 1064 nm laser in treating melasma is better than that of single-switch Qd Nd:YAG 1064 nm laser.3.The difference in therapeutic effect between auricular acupoint pressing and syndrome differentiation remains to be further studied.
